This book discusses the philosophical foundations of bioethics, with a particular focus on the tensions and potential dilemmas generated by the intuitionist meta-ethical commitments of the predominant normative theory, namely “the four principles approach.” This view is based on the prima facie norms of respect for autonomy (one ought to respect the autonomous choices of subjects of scientific research/patients), non-maleficence (one ought to refrain from inflicting harm), beneficence (one ought to do good and prevent, or remove, harm) and justice (one ought to treat people fairly). The tensions in applying these basic principles may lead to inaction in scientific experiments involving human subjects or to arbitrary applications of the norms in the art of caring. The problem can be made explicit in these terms: on the one hand, caring without respecting seems blind, degenerating into forms of paternalism when, for instance, the carer imposes her conception of the good life or a particular procedure on the cared-for; on the other hand, respecting without caring amounts to indifference or individualism when, for example, a person does not look after a vulnerable being properly. The initial hypothesis of this book, then, is that the concept of respectful care can be built up, working from an ethico-philosophical perspective, to be a leading notion capable of guiding our daily actions and bioethical practices.

This book fills an important gap in existing health care ethics literature by describing an egalitarian conception of moral respect which applies to autonomous and non-autonomous patients alike. It reframes questions about respect, from its target to the role that respect plays in our moral lives. Taking into account various forms of objectification, it suggests that the unique role of moral respect is to recognize a person as more than a mere object; to recognize them as an equally intrinsically valuable being who possesses dignity. Further, the book argues that respect is central to health care because medicine and experiences of illness are both inherently objectifying. Objectification is sometimes morally permissible, and other times morally troubling—a context of respect can help to distinguish between these situations. Because we can reduce others to mere objects in ways other than violating or denying their autonomy, the approach presented here can also accommodate non-autonomous patients directly without considering them as marginal cases.

In recent years, the triumph of autonomy has made paternalist interventions increasingly problematic. The value of a patient's right to self-determination and the practice of informed consent are considered supremely important in present-day health care ethics. This has left us with a situation in which paternalist medicine seems difficult to reconcile with respect for patient autonomy. This book offers a thorough reflection on the relationship between autonomy and paternalism, and argues that, from both theoretical and practical angles, the tension between these concepts is not as acute as it might seem. In long-term care, psychiatry, and care for the severely handicapped, the principle of respect for autonomy is particularly ill-suited. The meaning and application of the principle of beneficence to issues in health care is rarely clear or certain. Although the principle is frequently employed to justify a variety of actions and inactions, very little has been done from a conceptual point of view to test its relevance to these behaviors or to explore its relationship to other moral principles that also might be called upon to guide or justify conduct. Perhaps more than any other, the principle of benef icence seems particularly appropriate to contexts of health care in which two or more parties interact from positions of relative strength and weakness, advantage and need, to pursue some perceived goal. It is among those moral principles that Tom L. Beauchamp and James F. Childress selected in their textbook on bioethics as applicable to biomedicine in general and relevant to a range of specific issues ([1], pp. 135-167). More narrowly, The National Commission for the Protection of Human Subjects of Biomedical and Behav ioral Research identified beneficence as among those moral principles that have particular relevance to the conduct of research involving humans (2). Thus, the principle of beneficence is seen as pertinent to the routine delivery of health care, the discovery of new therapies, and the rationale of public policies related to health care.
