Chromosomal instability (CIN) is recurrent chromosome missegregation during successive mitotic divisions. Low rates of CIN can be tumor promoting, and contribute to the karyotypic heterogeneity observed in approximately half of breast cancers. High rates of CIN are tumoricidal, and represent the anti-cancer mechanism of microtubule poisons. Though microtubule-targeted therapies are widely used, approximately half of cancer patients are resistant to them. Novel strategies are needed to sensitize cancers, but progress has been hindered by limited understanding of the clinical mechanism of microtubule poisons and factors that dictate sensitivity. Pre-treatment CIN has recently emerged as a factor that sensitizes breast cancers to microtubule-targeted therapy. To characterize pre-treatment CIN, I analyzed a primary breast cancer tumor microarray, matched primary and metastatic samples, and patient-derived organoids. Contrary to expectations, chromosomes that fail to align at the metaphase plate, rather than chromosomes that lag behind the segregating masses of DNA, are the primary source of CIN in breast cancer. Metastatic breast cancers exhibit higher CIN than primary cancers, primarily due to increased misaligned chromosomes. Misaligned chromosomes are less likely to cause immune-activating micronuclei than other types of chromosome segregation errors. These findings demonstrate that misaligned chromosomes cause CIN in breast cancer and highlight the differential immunostimulatory effects of specific types of CIN. The microtubule stabilizer paclitaxel was recently shown to cause multipolar spindles without mitotic arrest at clinically relevant concentrations. Here I demonstrate that the microtubule destabilizers vinorelbine and eribulin similarly induce multipolar spindles without mitotic arrest at low doses. Maintaining multipolar spindles throughout mitosis leads to detrimental rates of CIN and cytotoxicity. However, focusing aberrant multipolar spindles into normal bipolar spindles reduces CIN and confers resistance to microtubule poisons. Early focusing provides greater resistance. Pharmacological methods to prevent focusing in vivo are not currently available. However, inhibiting the mitotic kinesin CENP-E can overcome resistance caused by spindle focusing by increasing CIN due to misaligned chromosomes. CENP-E inhibition could overcome primary resistance to microtubule poisons and enhance the clinical efficacy of these commonly used therapies.
