In bijna de hele Westerse wereld is de bevalling een medisch of technisch proces geworden. Het vindt plaats in het ziekenhuis bij de gynaecoloog, en de enige belangrijke uitkomst is eigenlijk de gezondheid van de baby. Nederland is daarop een eenzame uitzondering met onze thuisbevalling en zelfstandig werkende verloskundigen. Nog wel. De druk om ook hier zwangerschap en bevalling te medicaliseren, neemt toe. Simone Buitendijk stelt in haar oratie dat de stem van de zwangere vrouw te weinig wordt gehoord in de discussie over de beste zwangerschaps- en geboortezorg. Vrouwen moeten volgens haar de verantwoordelijkheid en zeggenschap over de baring terugkrijgen. Alleen als er aandacht komt voor belangrijke kwesties voor vrouwen, wordt het mogelijk geboortezorg op een menselijke, niet-gemedicaliseerde en veilige manier te organiseren. Wanneer domeinstrijd tussen zorgverleners en uitsluitend de cijfers van babysterfte het beleid gaan bepalen, wordt bevallen een mechanisch proces. Dat heeft volgens Buitendijk negatieve gevolgen voor zwangere vrouwen, hun partners en hun baby's.
