Cuando apareci› la primera edici›n, en 1949, la enfermedad por œlcera p?ptica se atribua por completo al estr?s, casi cada tumor que no fue extirpado resultaba en muerte, la cardiopata reumÿtica tena una amplia prevalencia y la hepatitis B y la infecci›n por VIH se desconocan. En los a?os intermedios, se identificaron tanto el origen infeccioso como la curaci›n para la œlcera p?ptica; los avances en el diagn›stico y el tratamiento hicieron posible curar 66% de los cÿnceres; la cardiopata casi desapareci›; la arteriopata coronaria ateroescler›tica se increment› y despu?s disminuy› (al menos en parte por la atenci›n de los factores de riesgo modificables); la hepatitis B y sus consecuencias, cirrosis y carcinoma hepatocelular, se tornaron prevenibles mediante una vacuna y el VIH, al principio visto como un azote uniformemente letal alrededor del mundo, ha llegado a ser una enfermedad cr›nica tratable. Durante ese mismo periodo, la cantidad de informaci›n requerida para la prÿctica eficaz de la medicina creci› de modo incontenible y las opciones de aprendizaje para los estudiantes, los residentes y los clnicos en ejercicio tambi?n florecieron para incluir mœltiples fuentes de informaci›n en formatos impresos y electr›nicos.
