This book portrays the performance of the health sector in Zambia using quantitative techniques. While there have been a number of health sector assessments in the country, they have relied on qualitative and anecdotal evidence for the most part. For the first time, this pubic expenditure review of the health sector brings together the results of three separate but related analytical efforts: multi-year national health accounts, a public expenditure tracking and quality of service delivery survey, and resource and impact modeling using the Marginal Budgeting for Bottlenecks software. This Public Expenditure Review (PER) for health in Zimbabwe is an analytical report developed by the World Bank Health, Nutrition and Population Task Team working with the World Bank's Macroeconomics and Fiscal Management Global Practice team, Zimbabwe's Ministry of Finance and Economic Development (MOFED) and Ministry of Health and Child Care (MOHCC), with input from development partners. The aims of the PER are to provide objective evidence to inform the allocation of resources to enable equitable, efficient, and sustainable health care provision and to highlight questions about health sector performance for policy makers and key stakeholders. The findings also aim to present Zimbabwe's health sector spending in an international and regional perspective, highlighting key issues for improving health outcomes. The PER is based on analysis of primary and secondary data as well as an extensive review of existing literature. The research was complemented by interviews with key stakeholders, and data collection from major development partners and domestic health funders, including private companies. Micro-level data from household and health facility surveys in May-August 2014 (primarily for the impact evaluation of the Results-Based Financing (RBF) program supported by the World Bank) complemented these scarce data.

The public expenditures review (PER) of the health sector was prepared in response to a request from the government of Egypt as part of a broader PER work that covers other sectors. This policy note is intended to inform government policymakers of the key findings from the PER and recommend strategies for improving the efficiency, equity, and sustainability of public financing in the health sector, with a special emphasis on expanding the coverage of social health insurance - a major plank in the health reform strategy announced by President Mubarak in July 2005. There are four sections in the report. The first section provides background information. The second section presents a summary of health outcomes, trends, and financing in Egypt; an analysis of inequities in the allocation and utilization of health resources; and an assessment of the efficiency of the delivery of health services. The third section summarizes existing government policies toward the health sector. The fourth section proposes strategies and recommendations for expanding social insurance coverage and enhancing incentives to deliver health services more efficiently and effectively.

This Public Expenditure Review (PER) is the result of collaboration among UNICEF, the World Bank Group, and Lesotho's Ministries of Health and Finance. Senior economists from the Clinton Health Access Initiative provided invaluable support in analyzing health expenditure data. The PER covers a five-year period, from fiscal years (FY) 2011/12 to 2015/16 (in Lesotho, the fiscal year runs from April 1 through March 30). The report describes and analyzes expenditure patterns in Lesotho's public health sector using multiple data sources. Focusing on the Ministry of Health and Social Welfare's (MoH) expenditure, the report identifies opportunities to improve the efficiency, equity, and effectiveness of financial resource use. To present a clear view of health expenditure, the report disaggregates total expenditure in the health system and describes its allocation across cost centers, districts (10 of them in Lesotho), levels of care, and health facilities (hospitals and primary health centers). Specific focus is on allocation of expenditure by economic classifications, and across outsourced health service providers.

This paper presents a framework for evaluating the level and composition of public expenditures, illustrated by sectoral and country examples. The paper illustrates how this framework can be applied to analyzing broad allocations of spending within and across sectors, drawing upon some key findings and country examples from major sectors (health, education, infrastructure). The report emphasizes six elements which should be an integral part of an ongoing exercise to evaluate public spending. Those elements include: (1) the aggregate level of public spending and deficit of the consolidated public sector must be consistent with the macroeconomic framework; (2) aggregate spending should be allocated to programs within and across sectors to maximize social welfare, including the impact on the poor; (3) the role of the government versus the private sector ought to be a principal criterion governing the choice of programs for public financing and provision--public expenditures should complement rather than substitute for private sector activities; (4) the impact of key programs on the poor, including their incidence and total costs, should be analyzed; (5) the input mix, or the allocations for capital and recurrent expenditures, should be analyzed in an integrated manner within programs and sectors; and (6) budgetary institutions should be analyzed to insure that the underlying incentive structure contributes to aggregate fiscal discipline, allocative efficiency and equity in the composition of spending, and technical efficiency in the use of budgeted resources. Fifteen tables (out of 49) and 5 appendices (out of 17) deal with education-related data.
