Asthma is a chronic respiratory disease that affects adults, children, and the elderly. Asthma has taken a toll on the economy of its sufferers, families, and health care providers. In the United States, approximately 23 million people have asthma. More than half of these individuals had at least one asthma attack in the previous year. This translates into an average of four days missed from school each year per asmthmatic child and two and a half days missed for work per asthmatic adult; 11 million outpatient medical visits; 2 million emergency room visits; nearly 400,000 hospitalizations; and approximately 4,000 deaths annually. In Maryland, approximately 12.9% of adults and 13.6% of children have a history of asthma. In 2007, asthma accounted for 9,800 hospitalizations and roughly 38,000 emergency department (ED) visits. Foreground: Students with asthma at a historically Black university were more likely to use the hospital ED than students with other chronic illnesses. A quality improvement (QI) needs assessment project conducted in the University Health Clinic (UHC) from February 2010 to April 2010 showed than 935 students with asthma visited the ED comapred to the 351 students with asthma that attended the clinic. The purpose of this project was to develop a Clinical Practice Guidelines (CPG) and a protocol for Evidence-Based Practice (EBP) for clinicians in the UHC. Goal: The short-term goal for this capstone project was to ensure that clinicians adhere to the Guidelines for the Diagnosis and Management of Asmthma CPG in the UHC for the treatment and management of asthma. The long-term goal is to decrease the frequent ED visits by students with asthma. Plan: The Change Theory, "unfreezing, movement, and refreezing" in Marquis and Houston was used as a theoretical foundation to accomplish the plan project. Rosswurm and Larrabee's EBP Model was used to guide the implementation of the capstone project. The plan included a pre- and post-test and a self-education package based on the CPG. The pretest assessed the clinician's base knowledge.

It documents the actions, assesses effects, and measures the quality and limitations of data for monitoring outcomes. The authors found that the implementation scored successes but resource limitations and organizational barriers curbed progress. They conclude that flexibility, monitoring, and training are the keys to implementing the guidelines. They also found that patient education needed improvement.

Asthma is a potentially fatal chronic disease responsible for over 1.8 million emergency room visits annually, and over five thousand deaths per year. The most rapid increase in asthma cases occurred in children under five years of age, with rates of disease increasing over 160 percent between 1980 and 1994. This quantitative, descriptive study utilized a retrospective medical chart review to determine if primary care providers adhered to aspects of the 2002 National Education Prevention Program Expert Panel Report: Guidelines for the Diagnosis and Management of Asthma. Data from 245 medical charts of nineteen primary care providers was collected and analyzed using simple and descriptive statistics. The results of this study showed that 97% of patients were not assigned an asthma severity/classification. There was no appreciable difference in the practice patterns between types of providers.

This Review sets out to propose a structure for the funding arrangements for the whole spectrum of health research, with the objective of obtaining the maximum benefit from research success and, where possible, eliminating duplication of effort. The Review found, however, that the UK is at risk of failing to reap the full economic, health and social benefits that the UK's public investment in health research should generate. There is no overarching UK health research strategy to ensure UK health priorities are considered through all types of research and there are two key gaps in the translation of health research: (i) translating ideas from basic and clinical research into the development of new products and approaches to treatment of disease and illness; (ii) implementing those new products and approaches into clinical practice.The Review also found that the wider funding arrangements for supporting translation of ideas from conception to practice could be more coherent or comprehensive and, where arrangements exist, they do not function well. The Review identified cultural, institutional and financial barriers to translating research into practice in the publicly funded research arena. But it also found that, in the private sector, the pharmaceuticals industry is facing increasing challenges in translating research into health and economic benefit. The Review has sought to make recommendations that will increase the translation of R&D into health and economic benefit for the UK, both in the public and private sectors. The Review recommends that the Government should seek to achieve better coordination of health research and more coherent funding arrangements to support translation by establishing an Office for Strategic Coordination of Health Research (OSCHR).

The Army Medical Department (AMEDD) has made a commitment to establishing a structure and process to support its military treatment facilities (MTFs) in implementing evidence-based practice guidelines with the goal of achieving best practices that reduce variation and enhance quality of medical care. The Quality Management Directorate of the Army Medical Command (MEDCOM) contracted with RAND to work as a partner in the development and testing of guideline implementation methods for ultimate application to an Army-wide guideline program. Three practice guideline demonstrations were fielded over a two-year period, in each of which participating Army MTFs implemented a different clinical practice guideline. All the demonstrations worked with practice guidelines that were established collaboratively by the Department of Veterans Affairs (VA) and Department of Defense (DoD).
