Decían los antiguos que la melancolía era una enfermedad y que era debida a que la bilis negra, una sustancia quimérica, que ascendía desde el bazo al cerebro y lo secaba, cosa que le pasó a don Quijote. En nuestros días, el modelo psicopatológico declara que muchos de los problemas como la depresión, las fobias o los intentos de suicidio son también una enfermedad. No existe ninguna evidencia científica de que estos problemas sean una enfermedad, ni de que estén causados por un supuesto desequilibrio de los neurotransmisores cerebrales como tampoco lo estaba la melancolía por un supuesto desequilibrio de la bilis negra. Declarar que una persona tiene una enfermedad porque se siente deprimida, tiene miedo a salir de casa u oye voces que le amenazan es una quimera de diagnóstico porque asigna una enfermedad inventada. Decir que una pastilla puede ser el tratamiento de la causa de esas experiencias es también un simulacro de tratamiento, porque la enfermedad que dicen curar es una invención, como fueron simulacro también las sangrías que se hacían con sanguijuelas en la yugular para evacuar la bilis negra que era la causa inventada de la melancolía. El libro hace de modo documentado, ameno y riguroso una crítica radical de la ortodoxia del modelo psicopatológico porque va a la raíz de esas experiencias vitales y señala cómo desde los modelos o paradigmas de la psicología puede ser desvelado y comprendido su significado.
