This is the first book published that discusses state of the art research regarding AMH in human reproduction and medicine. This unique and vital protein impacts a variety of fundamental processes within the ovaries and testes. Since its discovery in 1947, every medical student has learned about the role AMH plays in embryonic sexual differentiation. However, the last fifteen years have witnessed AMH emerge from the bench to the bedside, from a scientific curiosity to a useful diagnostic tool with clinical utility and therapeutic applications. In female reproduction, AMH has undergone a virtual renaissance after it was discovered in the human ovarian follicle and later to reflect ovarian reserve. Since that time, much has been learned regarding the role of AMH in female reproductive aging and a number of other gynecological conditions. This is reflected by a number of peer-reviewed publications on AMH in human adult females, which has increased exponentially over the past three decades. It is only in the last decade that AMH has become an important marker of ovarian response, guiding clinical decision making in assisted reproductive technology (ART) as well as a useful aid in the diagnosis of polycystic ovarian syndrome (PCOS). In male reproduction, AMH has become a diagnostic tool in the complex evaluation of infants with intersex conditions, including ambiguous genitalia and cryptorchidism. The chapters of this book bring together experienced investigators with a variety of research interests in different aspects of AMH. Each contributor has conducted clinical, translational and/or basic research in the field of AMH. Each has an appreciation of what is known and an aspiration for solving what remains to be discovered. A better understanding of AMH pathophysiology may lead to the development of pharmacological treatments using AMH agonists and/or antagonists for a host of reproductive challenges. These include: the possible retardation of ovarian aging and menopause; a chemoprotectant for fertility preservation prior to chemotherapy; a tool to better control ovarian response to ART and/or to mitigate the deleterious effects of polycystic ovary syndrome; and a treatment for endometriosis, adenomyosis and/or endometrial and ovarian cancers. This book enables the reader to review the past, understand the present and consider the future of this multifaceted protein in the fields of reproductive medicine and gynecology.

OBJECTIVE: AMH is associated with menopausal timing in several studies. In contrast to prior studies that were restricted to women with regular cycles, our objective was to examine this association in women with either regular or irregular menstrual cycles. METHODS: CARDIA is a longitudinal, population-based study that recruited adults ages 18-30 when it began in 1985-1986. AMH was measured in serum stored in 2002-2003. Natural menopause was assessed by survey in 2005-2006 and 2010-2011. RESULTS: Among 716 premenopausal women, median [25th, 75th] AMH was 0.77 [0.22-2.02]ng/dL at a median age of 42 [39-45] years. Twenty-nine percent of the women (n=207) reported natural menopause during 9 years of follow up. One-hundred and five virgin Holstein heifers were sampled pre-breeding for circulating AMH concentration and transrectal ultrasonography was performed to determine AFC and cyclicity status. Post-calving, heifers were sampled at 5-20 days in milk for AMH. At 45-60 days in milk, two-hundred and fifty-two primiparous and multiparous were enrolled and all animals were sampled for AMH concentration and transrectal ultrasonography was performed. Animals were assigned to an estrous detection or TAI breeding protocol and grouped based on pre-breeding circulating AMH. AMH was positively correlated with AFC, lactation number, age and milk-weight on sampling and breeding day (P0.0001). Conception risk to first service in cows and heifers was not impacted by breeding protocol or AMH category (P0.05). Delayed childbearing age, increased environmental pollutants accompanying modern life style, and an increase in inbreeding due to high demand for milk and meat yield have resulted in the decline in human and bovine reproduction respectively. Anti-Mullerian hormone (AMH) is expressed starting from 7-8 weeks of gestation in males and 20 weeks of gestation in females, in up to 100x higher concentration in males. AMH has been described as a non-invasive and accurate marker of ovarian quantitative fertility parameters in both humans and cattle. However, it is still unknown how AMH levels correlate to different fertility parameters in different cattle breeds; can AMH levels in the mother be used as a fetal sex marker during pregnancy in cows? In humans, can AMH collected in the single follicular fluid (mono FF), as well as granulosa cell (GC) AMH receptor 2 (AMHR2) be used as an embryo developmental potential marker? Here we present evidence of the higher levels of systemic and follicular AMH in Zebu compared to European cattle, corresponding to higher levels of AFC and oocytes. Cows pregnant with a male fetus, as compared to a female fetus, were observed to have a significantly different change in plasma AMH between day 35 and 135 during gestation. The placenta and cotyledon were found to express AMHR2 between day 38 and 80, however not significantly different between pregnancies with an opposite sex fetus. In the human study, systemic and average mono FF AMH, and average mono GC AMHR2 expression were significantly negatively correlated with the patient's blastocyst rate. When patients were divided into normal and high groups based on the their systemic AMH levels, the following was observed: normal systemic AMH level patients had a lower average FF AMH level and a higher blastocyst rate compared to the higher systemic AMH level patients. Also, in normal systemic AMH patients, blastocysts corresponded to follicles with a lower level of AMH, while in high systemic AMH patients blastocysts corresponded to follicles with a higher level of AMH.
