The anastomotic technique plays a predominant role in gastrointestinal surgery. A feared complication is leakage due to the sutures. Such leakage cannot be detected early; consequently, infection spreads locally and systemically. An anastomotic method is generally evaluated according to its rate of leakage, related to the localization, bur real scientific comparisons, i.e., controlled studies, are almost totally lack ing. The criteria of evaluation include the type of suture, the localization, the auxiliary technical tools, practicability, the different forms of wound healing, angiogenesis, and vascularization, among others. The postoperative criteria are complications shortly after surgery, such as bleeding, ruptures, and stenoses of the anastomosis. A standard comparison is made difficult by the variety of cytophysiological and biochemical factors that influence wound healing. In the comparison of larger series one must always take into account differences of auxiliary tools, strategies, and inhomogeneity of patients. A change of one auxiliary tool or of one strategy implies the modification of various target criteria. Often enough, however, one does not sufficiently consider the surgeon's most. important role.

When doctors first look at sutureless anastomosis devices created by new technologies, the most frequent comment is how amazingly easy it is to create a vascular anastomosis and, as a direct consequence, they believe anybody could play the role of the cardiovascular surgeon. Pessimistic c- diovascular surgeons, on their side, think that creating a machine capable of perfectly reproducing their core activity, which consists in making an- tomosis, will kill their profession. Actually, no medical specialty’s demise has been more often predicted and, at the same time, more greatly ex- gerated than that of cardiovascular surgery. According to its detractors, - ginning in the late 1980s with the angioplasty boom, continuing in the mid-1990s with the introduction of bare metal stents, and then more - cently with the introduction of drug-eluting stents, cardiovascular surgery has been on life support for nearly 15 years. This specialty’s claimed collapse has generally been attributed to recent advantages in percutaneous procedures and devices that give the oppor- nity to non-surgeon doctors to accomplish the surgeon’s work. The reality is different. Data suggest that cardiac surgery continues to survive and has not suffered the precipitous decline that was predicted. From the economical standpoint, CABG is much more of a mature market and the size of the opportunity remains impressive and relatively stable, with around 1,500,000 procedures performed annually worldwide, at a total cost approaching 2. 5 billion US$.

Biofragmentable anastomoses rings represent a fascinating concept: Stan dardization of anastomoses, secure technique in application, expulsion of the material without residuals. In addition, one single technique allows to perform the classical end to end, end to side, and side to side anastomoses in most areas of the gastrointestinal tract, without any auxiliary tool. This means a great advantage of practicability. Controlled studies evidenced that the rate of complications is very small for the intestinal tract. Therefore, it is important to establish the localization for present indication and contra indications. Two chapters especially deal with upper GI anastomoses in cluding esophagus anastomoses and with BAR anastomoses in inflammatory bowel disease. This is done by analysing the data and by an exchange of experience between the clinical research groups. The thrust forward into small dimensions with the aid of high magni fication (6-40 power) under the operating microscope marks an important development in almost all fields of surgery. Neurosurgery, ophthalmology and otorhinolaryngology have already integrated the binocular mono scope as an indispensable tool into their armamentarium. General surgery is also making increasing use of it-as is the case with transplantations of autogenous free intestinal grafts. A great "victory march of microsurgery", however, appears to be taking place in plastic and reconstructive surgery. Reimplantation of arms, hands, and especially digits are hardly thinkable today without the possibility of high magnification. New microsurgical anastomosing techniques (interfascicular neurorrhaphy) have also improved the func tional results after nerve injuries. Time-consuming reconstructive proce dures, such as the transfer of a distant flap with its physical and psycho logical burden, are being increasingly replaced by free flaps transplanted by microvascular suture techniques-the most popular free flap being the groin flap in which the superficial circumflex iliac artery and vein is reanastomosed to the recipient vessels. Nevertheless, the writer was sceptical when Dr. TSCHOPP set about with his ambitious plan to transplant a free autogenous rib graft in combination with its adjacent intercostal musculature. The evidence of the results obtained has given the lie to my scepticism. Dr. TSCHOPP has not only succeeded in securing the complete arterial and venous supply to the musculoskeletal graft at the recipient site but has also managed to preserve the functioning entity of nerve and muscle.
