Resumen: El presente documento se enfoca en una serie de análisis, reflexiones y propuestas conceptuales e instrumentales para abordar la gestión del cambio climático en los planes de desarrollo territorial. En este sentido, presenta la síntesis de un proceso de evaluación de las estrategias y mecanismos de planificación y gestión del cambio climático abordados en los territorios de la Ecorregión Eje Cafetero, ejercicio a partir del cual se logró construir un modelo conceptual de gestión que, a su vez, permitió concretar una herramienta para facilitar u orientar la incorporación de este tema en los planes de desarrollo territorial. Este instrumento consiste en un software denominado "KlimaTerraTorium", aplicativo que busca servir como instrumento para la democratización del conocimiento, la definición de prioridades y la descentralización de procesos de planificación y toma de decisiones, utilizando un mínimo de datos e información local para favorecer la gestión informada del cambio climático.
