It has been recognized for many years that cancers originating in the breast and prostate gland are frequently 'endocrine-dependent. ' Traditional thera pies included surgical endocrine ablative procedures or pharmacologic hor mone administration, both designed to antagonize the stimulatory effects of sex steroid hormones. In the past decade, several new treatment strategies for these tumors have emerged from basic studies in reproductive biology and mechanisms of action of steroid hormones. In some instances, these new treatments have elimin ated or reduced the need for major surgical ablative procedures or for toxic hormone therapy. The clinical role for other new treatments has not yet been clearly defined, although exciting preliminary data from recent clinical trials are now available. Thus, an objective review of the current status of these new therapeutic approaches is of interest. In this volume we have attempted to provide an in-depth review of both basic and clinical research involving several new treatment strategies for breast and prostate cancer. The first three chapters summarize preclinical and clinical studies of the luteinizing hormone-releasing hormone analogues, which can be used effectively to induce 'medical castration. ' Chapters 4, 5, and 6 review the rationale and clinical use of the compounds known collec tively as the aromatase inhibitors, which can also be used to suppress sex steroid hormone levels. Antiestrogen mechanism of action and its clinical implications for the design of innovative treatment approaches is considered in chapters 7 and 8.

Seventeen years ago Mr. HAYWARD came to work in the Breast Clinic at Guy's Hospital. At that time the influence of hormones on the progress of breast cancer was appreciated; the prescription of androgens and oestrogens in the treatment of the advanced case was well established, and the operations of adrenalectomy and hypo physectomy had recently been introduced. Nevertheless, the prevailing view of the nature of breast cancer was that it was a local malignancy which had to be eradicated before it spread too widely and only if that had occurred would the aid of hormones be enlisted. How these hormones worked was unknown, indeed, today, as Mr. HAY WARD points out, their mode of action is still unknown. What has happened, however, within the last seventeen years has been a change of emphasis in our views regarding the essential nature of this disease. Instead of breast cancer being considered primarily as a local fault spreading centrifugally, it is realised with increasing con viction that it is a generalised disease with local manifestations and that one of the principal, if not the principal, aberration from normality lies in the endocrine environ ment. That this philosophy is demanding increasing acceptance is due in a significant measure to Mr. HAYWARD'S own investigations. In this work he has had the devoted assistance of BRIDGET WOLFF of the Breast Clinic at Guy's Hospital and the expert collaboration of Dr. R. D. BULBROOK of the Imperial Cancer Research Fund.

From 11 to 15 July 1977 about 60 physiologists, endo crinologists, ecologists and other biologists from 14 countries convened at the University Montpellier for a symposium on Environmental Endocrinology. This meet ing was organized as a Satellite Symposium of the 27th International Congress of Physiological Sciences, Paris, 18-23 July 1977. This volume is a record of the com munications presented at the symposium. The objectives of the program were to examine the role of the endocrine system in a wide spectrum of adjustments and adaptations to changes in environmental conditions by various spe cies of animals, including man, and to promote an ex change of ideas among investigators who have approached these functions from diverse aspects. The diversity of the information and ideas communicated is great. Of necessity, they represent only an extremely modest se lection of the many facets of endocrine function in the interaction of animals with their environments. Be yond the usefulness of the communications individually, we hope that they collectively demonstrate the substan tial heuristic value of the concept of environmental endocrinology as it was perceived by the participants.
