Medical devices are the bread and butter from which health care and clinical research are derived. Such devices are used for patient care, genetic testing, clinical trials, and experimental clinical investigations. Without medical devices, there is no clinical research or patient care. Without life-adjusting devices, there are no medical procedures or surgery. Without life-saving and life-maintaining devices, there is no improvement in well-being and quality of life. Without innovative medical devices and experimentation, there can be no medical progress or patient safety. Medical devices and medical technology are used to create or support many different products and medical-surgical procedures. This volume on the regulation of medical devices in the European Union, with a focus on France, tackles a topic of interdisciplinary interest and significance for policymakers in countries around the globe. The EU regulatory regime is one of three global regional regimes, and medical products manufactured in EU countries are sold worldwide. As countries confront an aging population on a global scale, with associated increases in chronic diseases, physical handicaps, and multi-morbidity, there will inevitably be an increase in the demand for health services and, concomitantly, the use of medical devices in medical and surgical procedures. This will be the case regardless of whether services are delivered in hospitals, doctors' offices, or at home. The associated risks of a particular device will be the same whatever the country of origin for the device, or where the need occurs. Revolutionary medical advances increase diagnostic capabilities, but they increase the potential of harm and risks to patients. Medical technologies and devices are used ethically most of the time; yet they have the potential for unethical use when scientific medicine is elevated over human life and death. Assumptions that are taken for granted can be dangerous to a patient's health. That is why our understanding of appropriate and effective regulation of medical devices is significant to all people on all continents.

It is broadly accepted that we have moved (or are moving) to a knowled- based economy, characterized at least by two main features: that knowl edge is a major factor in economic growth, and innovation processes are systemic by nature. It is not surprising that this change in the economic paradigm requires new analytical foundations for innovation policies. One of the purposes of this book is to make suggestions as to what they should include. Underpinning all the chapters in this book is a conviction of the impor tance of dynamic and systemic approaches to innovation policy. Nelson (1959)^ and Arrow (1962)^ saw innovation and the creation of new knowl edge as the emergence and the diffusion of new information, characterized essentially as a public good. The more recent theoretical literature regarded the rationale for innovation policies as being to provide solutions to "mar ket failures". Today, however, knowledge is seen as multidimensional (tacit vs. codified) and open to interpretation. Acknowledging that the creation, coordination and diffusion of knowledge are dynamic and cumu lative processes, and that innovation processes result from the coordination of distributed knowledge, renders the "market failure" view of innovation policies obsolete. Innovation policies must be systemic and dynamic.

In the early stages of planning the Third International Conference in System Science in Health Care, the steering committee members, most of whom had participated in the first conference in Paris (1976) and the second in Montreal (1980), made some basic decisions about organization of subject matter. The earlier meetings had been very successful in bringing together specialists from the health professions and the traditional sciences. In addition to physicians and nurses, these were representatives of the disciplines of the behavioral sciences, system theory, economics, engineering, and the emergency fields of management science and informatics -all concerned with the development of health resources in a broad system context. The reported research and experience of the many disciplines represented had dealt with one or more of three concerns: 1) a major health problem, such as cardiovascular disease, or an important popUlation at risk, such as the elderly or children or workers; 2) some generic aspect of organization and decision making, including trial and evaluation ofinnovative health strategies; and 3) the methodology of research and analysis in system of health service. The challenge to the conference organizers lay in the eliciting and arranging of experiences in such a way that the health services could be seen as purposeful,living, evolving systems.

In contemporary global capitalism, the most powerful corporations are innovation or intellectual monopolies. The book’s unique perspective focuses on how private ownership and control of knowledge and data have become a major source of rent and power. The author explains how at the one pole, these corporations concentrate income, property and power in the United States, China, and in a handful of intellectual monopolies, particularly from digital and pharmaceutical industries, while at the other pole developing countries are left further behind. The book includes detailed empirical mappings of how intellectual monopolies develop and transform knowledge from universities and open-source collaborations into intangible assets. The result is a strategy that combines undermining the commons through privatization with harvesting from the same commons. The book ends with provoking reflections to tilt the scale against intellectual monopoly capitalism and arguing that desired changes require democratic mobilization of workers and citizens at large. This book represents one of the first attempts to capture the contours of an emerging new era where old perspectives lead us astray, and the old policy toolbox is hopelessly inadequate. Although co-design has been practised in new service and product development for some years, it has only recently begun to appear in the burgeoning field of social innovation.
