Fresh living Bacillus Calmette-Guerin (BeG), injected i.v. into (C57BI/ 6xDBA/2)FI mice, activated peritoneal macrophages rendering them highly cytotoxic for tumor cells in vitro. This cytotoxic activity w~s already maximal 14 days after injection of 1 mg of BCG and remained stable when 3 or 5 mg of BCG were given. At the same time spleen cells of the BCG-treat~d mice showed strongly depressed responses to the T-cell mitogens,PHA and Con A, irrespective of the dose of BCG injected. The inhibitory effect was shown to be mediated by suppressor cells which had characteristics of macrophages since they could be removed by carbonyl iron and magnet treatment and were adherent to plastic. In contrast to it was observed after injec tion of 1 mg of BCG, these suppressor cells alone did not account for the depression of T-cell re.sponses induced by higher doses of BCG. Nylon-nonadherent cell populations obtained from spleen cells treated with 3 or 5 mg BCG partially retained the inhibitory activity suggesting that suppressor T cells were also induced after injection of high doses of BCG. In contrast, the responses to the B-cell mitogen LPS of unfractionated and macrophage-depleted spleen cells were not affected or significantly enhanced depending on the dose of BCG injected.

Since cytotoxic drugs were first developed, their damaging effects on bone marrow have attracted considerable interest. We now know that the bone marrow is an organ with rapid cell renewal in which maintenance of the steady-state depends on a high rate of cell production, and that the bone marrow toxicity of cytotoxic agents can be explained by the perturbation of such steady-state conditions. Experimental researchers have analysed in detail the effects of cytotoxic agents upon defined hematopoietic cell populations to define the mechanism of action and the location within the cell cycle at which cytotoxic drugs exert their cytocidal action. From such studies, a classification of cytotoxic agents according to their action upon cells during different phases of the cell cycle has been proposed [80-82]. Others have used the acute perturbation of the hematopoietic systems, caused by application of single high doses of cytotoxic agents, to analyse the patterns of depletion and reconstitution of the various hematopoietic compartments in an effort to shed light on the regulatory mechanisms that are responsible for the delicately balanced cell production of the hematopoietic cell renewal systems. A great deal of information on the structure of the hematopoietic systems has come from such studies, and kinetic parameters of these systems have been defined. To clinicians, the hematopoietic toxicity of antineoplastic agents has been an undesired though unavoidable side effect of almost all cytotoxic drugs introduced into clinical practice.

Problem: Most cases of fallopian tube, ovarian, and primary peritoneal serous cancer (pelvic serous carcinoma) are diagnosed at an advanced stage, and the patients present with widespread tumor throughout the peritoneal cavity and possibly the liver parenchyma or lungs. The standard of care for initial treatment is cytoreductive surgery where all visible disease is removed. In order to obtain complete or maximal resection of disease, upper abdominal procedures, including splenectomy, may be necessary. The purpose of this literature review is to discuss studies that support optimal cytoreduction including splenectomy in pelvic serous cancer patients and aim to identify hematologic changes after splenectomy that help to differentiate infection from the normal post-splenectomy hematologic response. Methods: A Medline search using the online resources of the Weill Cornell Medical College Library was conducted to identify articles pertaining to ovarian cancer cytoreduction, hematologic changes after splenectomy in ovarian cancer patients and non-ovarian cancer patients, and postsplenectomy infections. Results: All studies analyzed for this review support extensive cytoreductive surgery, including splenectomy if necessary, as initial treatment for pelvic serous cancer. Information to help determine if leukocytosis in the postsplenectomy patient is indicative of infection is limited in the pelvic serous patient population with only one study touching on this topic. Conclusions: The effects of splenectomy in the trauma patient population have been studied more thoroughly, but only one review in the literature has investigated these changes in the gynecologic cancer patient population. Differentiating the hematologic changes associated with removal of the spleen from the hematologic changes that indicate an infection is important in the management of the immediate postoperative pelvic serous cancer patient.

As a medical student in the 1930s, I remember well that a very clear line of distinction was drawn between physical illness and mental illness. Physical illness resulted from pathology which could be identified and studied in detail and the cause could often be deduced. Treatment thus was often specific and based upon a proper appreciation of underlying basic science. Mental illness, on the other hand, could not, it was believed, be based upon identifiable changes in basic science. It was literally 'all in the mind' and, occurring in the absence of causes that could be identified, analysed and remedied, had no treatment that could in any way be called specific. N ow it is all too easy, in Medicine, for a doctor to become so immersed in a specialty that he fails to notice how his colleagues in other specialties are faring and, through laziness, to miss advances even of fundamental importance in fields other than those in which he himself works. As a consultant surgeon, after W orId War II, although I realized the importance of keeping in touch with those medical specialties which appeared to have a common interface with my own surgical interests, such as general medi cine, gastro-enterology, hepatology and endocrinology, it did not occur to me that there was any pressing need to ask what was new in Psychological Medicine.
