Abstract: Abstract of thesis entitled Donor Perspective of Right Lobe Adult-to-Adult Live Donor Liver Transplantation submitted by See Ching CHAN for the Degree of Master of Surgery at the University of Hong Kong in August 2005 Donors of adult-to-adult live donor liver transplantation (ALDLT) undergo a very major surgical operation which carries a 0.5% mortality, 20% morbidity, and has unknown long-term consequences. Despite these caveats, ALDLT is now an accepted treatment modality and has saved the lives of thousands of patients with end-stage liver disease of various etiologies and small unresectable hepatocellular carcinoma. The rapid adoption of ALDLT is associated with the scarcity of deceased donor liver grafts. However, pure demand from the recipient should not progress to ALDLT. To a minimum, it is the potential donor's perception of the expected recipient outcomes and the donor costs which substantiates such voluntarism. Nonetheless, the potential donors are seldom trained to make such decisions. We as guardians are obligated to guide the potential donors and the potential recipients throughout the process. Conversance of contemporary donor and recipient outcomes is therefore mandatory. The ultimate aim of this thesis was to provide a synopsis of the endeavors toward the holistic care of the highest standard for donors of right lobe ALDLT. This was done through: 1) optimum donor workup, 2) accurate graft size requirement estimation, 3) safe donor right hepatectomy, 4) quantifying quality of life of donor, 5) documentation of long-term biological consequences of donor surgery, and 6) establishing standards for donor right hepatectomy. In a cohort of 399 potential donors for ALDLT, 32.1% (128/399) underwent donor operation. Dropouts because of incompatible blood group, positive hepatitis ii serology, and voluntary donor withdrawal were 52.6% (210/399). Only 11.6% (22/189) were determined unsuitable for psychological or anatomical reasons as revealed in the further workup. Careful and expeditious initial donor workup minimized wastage which was not related to blood group or serological reasons. Adequacy of graft size is a prerequisite for recipient survival. Based on the right lobe graft weight on the back table and the volumetric analysis of donor computed tomography, a formula of liver weight estimation by body weight and gender for the local population was derived: liver weight (g) = 218 + body weight (kg) x 12.3 + gender x 51 (female = 0, male = 1). The technicality of donor right hepatectomy contributes directly to the donor safety and recipient success. Based on donor computed tomography and operative data, preservation of segment 4b hepatic vein with substantial venous drainage into the middle hepatic vein for donors was validated as clinically important. Even though the liver has remarkable regenerative ability, right lobe donors operated over two and a half years or earlier had statistically significant decrease in the white cell count, and elevation of serum creatinine, alanine aminotransferase, aspartate aminotransferase, and bilirubin at follow-up. Abstract: ABSTRACT of thesis entitled: "Application of living donor liver transplantation to adult recipients in Hong Kong" Submitted by Dr. Chung-Mau LO for the degree of Master of Surgery at the University of Hong Kong in July 1998 Despite the acceptance of organ transplantation from brain-dead donors in Hong Kong, the availability of cadaveric liver grafts is severely restricted by Chinese religious and cultural traditions. For 3 years since the initiation of the liver transplant program at Queen Mary Hospital in October 1991, only nine cadaveric grafts were available for transplantation and the liver donor rate in 1993 was 0.6 per million populations per year. With the application of living donor liver transplantation (LDLT) to pediatric patients, the disparity between the supply and demand for donor organs was in adult recipients. In particular, high-urgency candidates who required emergency transplantation had a pretransplantation mortality of nearly 100%. A review of the literature showed that the extension of LDLT to adults had not been made with success. The aim of this thesis is to develop a strategy to overcome the problem of organ shortage by evaluating the possible application of LDLT to adult recipients in Hong Kong. From July 1994, 5 adult patients underwent LDLT using a full left lobe graft. Four of the operations were performed as an emergency for high-urgency candidates. The donor-to-recipient body weight ratio ranged from 104% to 139%. Although the body weights and heights of all the donors were greater than those of the ncorresponding recipients, the weight of the left lobe graft amounted to only 31% to 54% of the recipient's expected liver weight. Two recipients developed early graft dysfunction and died. The other 3 recipients (60%) survived after a period of cholestasis. From May 1996 to January 1998, a new technique of liver transplantation using an extended right lobe graft from a living donor was applied to 9 high-urgency adult patients. The body weights of 5 donors were less than those of the corresponding recipients and the lowest donor-to-recipient body weight ratio was 62%. Nonetheless, the ratio of the graft weight to the recipient's expected liver weight (graft weight ratio) was more than 40% in all except one cases and 8 of 9 recipients (89%) survived with the original graft at a median follow-up of 16.5 months. The technique has overcome the problem of donor-to-recipient size match and extended the success of LDLT from children to adults of virtually all sizes. To establish the criteria for selecting the type of donor operation, a retrospective analysis of 25 LDLTs using 9 extended right lobe grafts and 16 left lobe or left lateral segment grafts was undertaken. For the donor, an extended right lobectomy carried a higher risk of transient postoperative liver dysfunction, a prolonged hospital stay, and possibly increased morbidity. Overweight and obesity have become urgent global health issues in recent decades. Globally the number of overweight children under the age of 5 years has increased from 32 million in 2000 to 41 million in 2014 corresponding to an increase in prevalence from 5.0% to 6.1%. It is estimated that at the current pace by 2020 some 9% of all children under 5 years will be overweight. Furthermore an increase in adult obesity prevalence has been observed in all countries and globally the prevalence of obesity among adults has doubled from 1980 to 2014 from 5% to 11% for men and from 8% to 15% for women. Overweight and obesity were estimated to account for 3.4 million deaths annually and 93.6 million disability-adjusted life years (DALYs) 1 in 2010. The burden of overweight and obesity is inequitably distributed and affects some population groups and geographical areas more than others based on their social characteristics which are also inequitably distributed. Vulnerability to overweight and obesity might depend on for instance urban or rural residence socioeconomic status ethnicity or the geographical area where people live and their nutritional status in the first 1000 days of life. This report intends to assist policy-makers in the World Health Organization (WHO) Western Pacific Region by contributing to a better understanding of the unequal distribution of overweight and obesity in the Region and by providing policy options to address the social determinants of overweight and obesity. Identifying vulnerable population groups or areas can help policymakers programme managers and other actors to improve programme targeting and increase the effectiveness and improve the health and well-being of the most vulnerable.
