For patients and their loved ones, no care decisions are more profound than those made near the end of life. Unfortunately, the experience of dying in the United States is often characterized by fragmented care, inadequate treatment of distressing symptoms, frequent transitions among care settings, and enormous care responsibilities for families. According to this report, the current health care system of rendering more intensive services than are necessary and desired by patients, and the lack of coordination among programs increases risks to patients and creates avoidable burdens on them and their families. Dying in America is a study of the current state of health care for persons of all ages who are nearing the end of life. Death is not a strictly medical event. Ideally, health care for those nearing the end of life harmonizes with social, psychological, and spiritual support. All people with advanced illnesses who may be approaching the end of life are entitled to access to high-quality, compassionate, evidence-based care, consistent with their wishes. Dying in America evaluates strategies to integrate care into a person- and family-centered, team-based framework, and makes recommendations to create a system that coordinates care and supports and respects the choices of patients and their families. The findings and recommendations of this report will address the needs of patients and their families and assist policy makers, clinicians and their educational and credentialing bodies, leaders of health care delivery and financing organizations, researchers, public and private funders, religious and community leaders, advocates of better care, journalists, and the public to provide the best care possible for people nearing the end of life.

This treatise had its origins in the authors' strong opinion that the discovery of new drugs, especially of innovative therapeutic agents, really does not happen as a spontaneous sequel to investiga tive research, no matter how penetrating such research may be. Rather, it seemed to us that the discovery of innovative therapeutic agents was a very active process, existing in and of itself, and demanding full attention-it was not simply a passive, dependent by-process of investigative research. And yet, many researchers some close confreres of the authors, others more distant-believed otherwise. We felt that their view reflected unrealistic thinking and that reality probably lay closer to what Beyer" maintained: We are taught to believe that if we can understand a disease it should be easy enough to figure out, say, the molecular configuration of a definitive receptor mechanism somewhere along the line and to design a specific drug . . . . And so we start out to understand the disease but never get around to doing much about therapy. The authors very soon realized that there was essentially no quantitive information available on just where and how innovative therapeutic agents were discovered. There were only anecdotal accounts, and these were able to be selected and presented in ways that could be used to defend any point of view.

Simons (1982) recently devised a method allowing the measurement of the steady 2 2 state internal Ca+ concentration. Cells are suspended in media of different Ca+ con 2 2 tent whose Ca+ concentration is monitored by a Ca+ -selective electrode. When the cells are lysed (by digitonin) there is an upward or downward deflection of the elec 2 trode signal. At the point of zero deflection, the cellular Ca+ concentration equals that 2 of the medium. The result is, that in fresh human red blood cells the Ca+ concentra tion is ;;;; O.4,uM (this is an upper estimate; the true value may be considerably lower).

Cardiac output has always been a subject of interest to both clinicians and researchers in different branches of medicine and surgery. In the last decade more attention has also been paid to its application in pediatrics, neonatology, fetal medicine and pregnancy. Better understanding of the peripheral circulation has provided more insight into the patholophysiology of different diseases. Many cardiac and non-cardiac disorders affect cardiac outputs. Monitoring of the changes in cardiac output is also important in the acutely ill patient. There are several methods to measure cardiac output, each with advantages and pitfalls. This book deals with all relevant aspects of cardiac output in eight parts: part one describes the methods of measuring cardiac output and a comparison between the catheterisation based and the noninvasive techniques, while part two describes the changes in cardiac output due to physiological causes. Part three describes cardiac output in cardiac diseases and systemic hypertension. Cardiac output in acutely ill patients is discussed in part four. Effect of cardiac medications, temporary atrial pacing, permanent pacing, pharmacologic stress testing and anesthesia are covered in detail in part six, while changes in cardiac output in noncardiac diseases are described in part seven. Finally great attention has been paid in part eight to the regional circulation including cerebral, coronary, skeletal and splanchnic circulations. A separate chapter discusses in detail the dynamics of blood flow. This book will be useful both to the cardiologists as well as to physicians in other fields of surgery and medicine and to their trainees. The first volume of Professor Woodman's edition of, and commentary on, Velleius Paterculus was published in the Cambridge Classical Texts and Commentaries series in 1977. This is the second volume to appear, covering Velleius' narrative of Julius Caesar and Augustus, down to 19 B.C. Velleius' history was first published in A. D. 30 and is being increasingly regarded as an important source for Roman history. Professor Woodman's aims have been the same as in his first volume: to establish the text, or at least to indicate where it is unreliable, and to explain the nature and meaning of the narrative. Thus his commentary is primarily textual, linguistic and stylistic, to be used by those who want to read Velleius, whether their interests are literary, historiographical or historical. It is the first commentary of its scale and scope since the beginning of the nineteenth century.
