Excerpt from Medico-Chirurgical Transactions, 1894, Vol. 77: Second Series, Volume 59 You have already heard in the Report of your Council the steps which were taken upon the death of the President, and how it happens that I have the honour of addressing you today. Those who were present at the last Annual Meeting, or who have read the address the President then delivered in the last volume of our Transactions, ' will remember that in it he reverted once again to a subject which has at intervals been brought before the Society - the organisation of a Royal Academy of Medicine out of the existing Societies 3 and he concluded by saying, I hope at no distant date to have the opportunity of suggesting for serious consideration the outlines of a plan for the creation by federation of a Royal Academy of Medicine and Surgery. If he had been spared to address you to-day, I think we should have heard from him a further development of his views on this most important proposal 3 for during last autumn, very shortly before his return to town, Sir Andrew Clark had a long conversa tion with me on this subject, and it is very greatly to be regretted that by his untimely death any plan for carrying out this most desirable scheme should have lost the benefit of his great in uence, clear judgment, and powers of organisation. Excerpt from Medico-Chirurgical Transactions, 1888, Vol. 53 Typhlitis, or inflammation of the cacum, is not to be denied. Ulcers - and notably stercoral ulcers, due to the impaction of faeces in the bowel - are not uncommon in the cacum. These may lead to perforation, to peri typhlitis, and possibly to inflammation of the connective tissue that is not far distant. The overwhelming influence of the appendix has been well demonstrated by Fita1 With, Matterstock', and others. The appendix is a part of the human body that is in reality obsolete, and exists as an organic anachronism. It is the deformed relict of the elongated cacum of lower mammals - of the capacious cacum of the rodents, and the remarkable blind gut of the marsupials. It would appear to be functionless and to be perishing (as do many obsolete organs) by a pathological process. So high is the mortality in typhlitis, and so common is the disease, that the appendix may be conceived to take some share in evolution, since in the question of the survival of the fittest an advantage would attach to the being whose appendix was abortive or had been obliterated by early disease. The operation, how ever, has so very rarely been performed for recurrent carcinoma after removal of the breast, and involves such different questions, that I venture to bring the following case under the notice of the Fellows. S. T aged 53, single, a nurse, was admitted to St. George's Hospital in December, 1894. For about two years she had noticed in the left breast a lump which gradually increased and was painless.
