This book does not pretend to be a comprehensive textbook of nephrology, nor is it a guide to the management of renal disease, as space does not allow the description or discus sion of investigation and treatment. Its intention is to illustrate the diverse diseases which present to nephrologists. Since the key to understanding the effects and progression of a disease are to know and com prehend its pathology, I have approached the subject not by discussing presenting symptoms of single diseases in different sections, which would lead to much repetition, but by describing the disorders according to the mechanisms which cause them. To illustrate the text I have chosen the most immediate clue which usually appears in a given dis ease, which may therefore be a clinical sign, a radiograph or the histological appearance of a renal biopsy. This may arise directly from injuries to the kidneys themselves despite their protected position (Figure 1). Trauma can also cause acute renal failure by damaging the renal arteries, which may be actually avulsed from the kidneys, or stretched at the time of impact, with tearing of the arterial wall and secondary thrombosis. Figure 2 shows the renal arteriogram of a young man who presented with anuric acute renal failure after a motorbike accident. Only the upper pole of the right kidney is Figure 1.

Immunologic problems in renal disease Rnd metabolic and medical aspects of urolithiasis were the two subjects of the Congress, which was attended by numerous invi~ed speakers and participants. The first part was introduced by A. Vercellone (Torino), who discussed the major steps in the development and the present per spectives of nephrology, a relatively newly born science, recognized as such only in 1960. He called attention to the great significance of our present knowledge of the immunologic mechanisms (circulating immune complexes or in situ mounting, cellular immunity, activation of complement) which are involved in the pathogenesis of glomerulo nephritis.

Renal Failure Prevention and Treatment in the 19808 It appears logical to juxtapose in this volume prevention-low cost and nonmorbid-with uremia therapy, which is very morbid and very high cost. Treated uremic patients constitute an important, complex, and demanding group of survivors of a formerly universally fatal disease. Throughout the developed nations of the world, an increasing fraction of the health care budget is devoted to sustaining lives by dialytic therapy and renal transplantation. In the United States, for example, patients in renal failure comprise 0.2% of those eligible for support by Medicare, but consume 5.0% of the Medicare budget. Economic stresses in funding kidney patients have, in some countries such as Great Britain, forced a return to restrictive selection policies abhorrent to empathetic physicians. For third world residents, attention to nutrition, sanitation, and infections such as malaria must take a higher priority than costly uremia therapy. Thus the solution of one problem (retarding death from uremia) created several equally vexing other dilemmas (who should be treated and at what cost?). While sociologists, economists, and ethicists struggle with the new field of psychonephrology,1 a group of investigators and clinicians convened to examine medical aspects of long-surviving treated uremic patients. These proceedings represent the first American analyis of those unique patients who have lived for ten or more years beyond what would have formerly been certain death in uremia.
