Although officially 'dismantled', the National Programme for IT in the NHS continues in the form of separate component programmes which are still racking up big costs. The original contracts with CSC totalled £3.1 billion for the setting up of the Lorenzo care records system in trusts in the North, Midlands and East. Despite the contractor's weak performance, the Department of Health is itself in a weak position in its attempts to renegotiate the contracts. It couldn't meet the contractual obligation to make enough trusts available to take the system. We still don't know what the full cost of the National Programme will be. The Department's latest estimate of £9.8 billion leaves out the future costs of Lorenzo or the potential large future costs arising from the Department's termination of Fujitsu's contract for care records systems in the South of England. Parliament needs to be kept informed not only of what additional costs are being incurred, but also of exactly what has been delivered so far. The Department estimates £3.7 billion of benefits to March 2012, just half of the costs incurred. There is still a long way to go before government departments can honestly say that they have learned and properly applied the lessons from previous contracting. Given the Department's track record with the National Programme, it is very hard to believe that the paperless NHS towards which the Department is working has much chance of being achieved by the target date of 2018

The Department for Transport has approved expenditure of over £11 billion between 1998 and 2021 for the development of new and existing trunk roads and motorways by the Highways Agency, and just under £1.7 billion on major road schemes proposed and developed by local authorities in five year Local Transport Plans. Following on from a NAO report on this topic (HCP 321, session 2006-07; ISBN 9780102944600) published in March 2007, the Committee's report examines the steps taken by the Department for Transport and the Highways Agency to improve value for money and oversight of the roads programme and contracting methods and project management capability. By September 2006, the Agency's 36 completed schemes in the Targeted Programme of Improvement cost 40 per cent more than estimated initially, and for schemes still to be completed, latest forecasts indicate that final costs could be 27 per cent more than original estimates. The main causes for costs exceeding estimates are increases in construction costs, higher than forecast land prices and compensation to landowners, inflation and changes in the scope of the project. The report finds that the DfT has not been rigorous enough in its oversight of the Agency's delivery of major road schemes, allowing it too much latitude on delivery and cost plans, and has failed to monitor in-year expenditure against progress and delivery milestones. The Agency is overly reliant on consultants for project management expertise and needs to develop its in-house capability.

In October 2003, the Department of Health (the Department) agreed a new national contract for NHS medical consultants in England. This report examines the contract negotiation; the cost implications; the effectiveness of the implementation process; and the extent to which the expected benefits for patients and the NHS had been realised. It was intended that employers would get greater control and management of their consultants' workload, and patients would benefit from a more flexible and responsive service. The Department hoped to reward consultants who made the biggest contribution to NHS work and reduce the average number of hours worked, in exchange for increased productivity. These benefits were dependent on a mandatory and rigorous process of workload planning for individual consultants (job planning). The implementation of the contract was rushed and the NHS has yet to see many of the intended benefits. Over the first three years, the Department allocated an additional £715 million to NHS trusts which was £150 million more than originally estimated. Although consultants' pay has, on average, increased by 27 per cent (from £86,746 to £109,974) and their working hours have decreased, there are no measurable improvements in productivity. The Department has succeeded in increasing the number of consultants working in the NHS, from 28,750 in October 2003 to 31,990 by September 2005, but the number of hours consultants work in private practice has neither increased nor significantly decreased. Other intended benefits have not been realised: for example the proportion of time consultants spend on direct clinical care is less than intended, and the contract has not been used to extend and develop new services for patients.
