ObjectivesRed cell distribution width (RDW), a measure of erythrocytes size variability, is routinely measured in every blood count. Recently, RDW has been shown to be a prognostic marker for mortality particularly in cardiovascular disorders and recently in the critically ill. The aim of this study was to investigate if RDW has the potential to predict not only mortality but also organ failure in an independent, large and unselected population of intensive care unit (ICU) patients. The exposure of interest was RDW as a predictor of mortality and organ failure in the critical care population. Design: One center observational study.Setting: Intensive care unit of the University Hospital of Antwerp, Belgium.Patients: We retrospectively studied a large population of 11957 adult patients who received critical care between 2007 and 2013.Interventions: None.Materials and Methods:A series of hematological markers of the first day of admission (RDW, HDW, MPV, MCV, MCH, MCHC), demographic data, validated scores for pathological severity (SAPS-3) and nurse workload (TISS), markers of inflammation as well as a large number of variables for multiple and single organ failure: the SOFA score, markers of renal function, respiratory insufficiency, liver function and global hypoperfusion. Results: There was a stepwise increase with increasing quintile of RDW for the SAPS-3 score, the length of stay in the ICU, and the mortality rate both in the ICU and in the hospital. An increasing gradient across RDW quintiles was also observed for the parameters for renal function. Moreover patients with a higher RDW quintile had a higher change of achieving a higher SOFA-max during their stay in the ICU. This statistically significant increase was also seen for each individual SOFA category which describes single organ failure. Moreover a higher RDW quintile was associated with a longer duration of mechanical ventilation, a higher maximal lactate level and a lower arterial pH value. This stepwise association was robust, but not unique for RDW. Comparable trends were seen by analyzing quintiles for another marker of cell width variability i.e. the mean platelet volume (MPV). Elevated RDW during the first day of admission was associated with a significant increase in mortality rates irrespective to hemoglobin levels. Univariate logistic regression suggests that elevations of RDW on admission are associated with a higher risk of mortality with an Odds ratio of 1.564 (CI 1.484-1.648, P

BackgroundRed Cell Distribution Width (RDW) could be a risk factor for developing various chronic diseases, and seems to be a prognostic marker in patients with cardiovascular disease (CVD) or cancer. Our aim was to explore the association between RDW and all-cause and cause-specific mortality in a general population.MethodsRDW was measured in 27,063 participants (aged 45-73 years) from the population-based Malmu00f6 Diet and Cancer cohort. After a follow-up of 19.8u00b15.5 years, Cox proportional hazards regression analysis was used to study the relationship between RDW and all-cause and cause-specific mortality, with adjustment for potential confounders.ResultsA total of 9388 individuals (4715 men and 4673 women) died during the follow up. Subjects with higher RDW had a significant higher risk for mortality, which remained significant after adjustment for potential confounders (HR, 4th vs. 1st of sex-specific quartile: 1.34, 95%CI:1.24-1.45). RDW was significantly associated with cancer mortality (HR, 4th vs. 1st: 1.27, 95%CI:1.12-1.44), CVD mortality (HR:1.39, 95%CI:1.21-1.59), and respiratory disease mortality (HR:1.47, 95%CI:1.06-2.03).ConclusionsHigh RDW is associated with increased all-cause and cause-specific mortality. We propose that high RDW is a significant, but non-specific marker of mortality risk in the general population.

Inherited as an autosomal recessive, the WIC-rdw rat exhibits congenital dwarfism (rdw). Genetic linkage analysis revealed that the rdw gene locus was identical to the thyroglobulin (Tg) gene locus. Complete sequencing of both wild type and the WIC-rdw rat Tg cDNA2s revealed a single missense mutation G6958C, causing a G2320R amino acid substitution at a position that is highly conserved. Further, transient expression of intact Tg cDNA containing the rdw mutation in COS-7 cells showed no detectable Tg in the secreted media confirming a defect in the export of rdw Tg. Despite the many similar features to other animal models for congenital hypothyroidism, the one major difference was that the rdw rat thyroid glands displayed marked hypoplasia. To examine the role of multiple ER chaperones in the processing of both wild type and rdw Tg, we followed their interactions by co-immunoprecipitation after pulse-chase. In COS-7 cells transfected with wild type Tg, the kinetics of dissociation of the chaperone BiP from Tg closely followed the kinetics of folding and ER exit of Tg, whereas, BiP association with the mutant Tg2s did not diminish over the same early chase periods. The oxidoreductases ERp72 and PDI but not ERp57 displayed little or no interaction with wild type Tg but did display significant stable binding to the mutant Tg. At long chase times up to 24 hrs, it was observed that majority of intracellularly retained mutant Tg was associated with BiP, ERp72 and PDI. These interactions were found to be both covalent and non-covalent. The classic non-covalent Tg 6 chaperone interaction diminished in parallel with the intracellular degradation of the rdw-mutant Tg but the covalent interaction with the redox chaperones remained constant throughout the chase period. During extended periods of chase, the covalently linked fraction remained unchanged. Investigations of both wild type and rdw rat thyroid tissue sections revealed the presence of foci that were positive for apoptosis. Thus, it is possible that the loss-of-function rdw mutation that results in accumulation of mutant Tg in the thryocyte ER may also be responsible for cellular toxicity leading to apoptosis, accounting for the observed thyroid gland hypoplasia.

Abstract : Abstract: The prognosis of breast cancer occurs in young women is usually poor. Red cell distribution width (RDW), 1 of many routinely examined parameters, has recently been proposed as a prognostic marker in solid tumors. The aim of our study was to assess the predictive value of RDW for survival in young women with breast cancer. We reviewed 203 consecutive young female patients (under 40) with invasive breast cancer diagnosed at the First Affiliated Hospital of Wenzhou Medical University between January 2008 and December 2012. Preoperational RDW, clinicopathological information, and prognostic data were collected. RDW levels were divided into 2 groups: 161 patients with low RDW (≤13.75%) and 42 patients with high RDW (>13.75%). Clinicopathological differences between the 2 groups were calculated by chi-squared test and Wilcoxon rank-sum test. Kaplan–Meier survival analysis and Cox proportional hazard regression analyses were used to examine the effect of RDW on survival. We found that high RDW was significantly associated with larger tumor size ( P = 0.002), positive lymph node metastases ( P = 0.011), and advanced stages ( P = 0.004).
