Introduction:TACE is established tumour control measure in patients with hepatocellular carcinoma (HCC) prior to liver transplantation (LT). Loco-regional effects of TACE may adversely affect the technical aspects of LT, and hence implications for graft selection.Methods:Retrospective cohort study involving patients with HCC that underwent TACE before LT (TACE group) between January 2011 and December 2015. The outcome of this group was compared with patients that underwent LT for HCC but without preoperative TACE (NoTACE group). Post-transplant surgical morbidity was compared according to Clavien classification.Results:59 cases included in the TACE group were compared with 123 cases in the NoTACE group. TACE group had higher prevalence of ischaemia related native collateral damage (11.86% vs 0%; p=0.002), native hepatic artery occlusion identified at surgery (5.1% vs 0%;p=0.033) and adhesions between the necrotic tumour to adjacent structures (14.75% vs. 3.15%; p=0.005). Surgical morbidity rate was significantly higher in the TACE group considering Dindo-Clavien grade 2 complications (64.4% vs 47.2%; p=0.029), however this was not found for grade u2265 3 (45,8% vs 58,5%; p=0,105). Patients in the TACE group transplanted with grafts from donors after cardiac death had 8.7% 90-day mortality and 56.5% major complication rate compared to 6.7% and 65% for the NoTACE group (p=0.719 and p=0.475; respectively).Discussion:HCC patients undergoing TACE while in the LT waiting list may lead to complications related to the procedure experienced in the laparotomy, nevertheless it has not shown repercussions on the mortality and only partial impact at morbidity, even considering the use of marginal grafts.

The systemic inflammatory response syndrome (SIRS) describes the immuneresponse to an insult that is inappropriate for a patient?s biological needs. Tumournecrosis factor-? (TNF) is one of the principal pro-inflammatory cytokines thatmediates SIRS. TNF-? converting enzyme (TACE) is responsible for the ectodomaincleavage of numerous immunologically relevant substrates including TNF, both TNFreceptors and the cellular adhesion molecule L-selectin. The biological significanceof TACE lays in its ability to control the availability of these key mediators of theimmune response in their membrane-bound and soluble forms. Little is knownconcerning the regulation of TACE-mediated ectodomain shedding and it contributesto disease pathophysiology. Our objective was to investigate the regulation of TACE catalytic activity andexpression during acute inflammation in vitro and in vivo. Our aims were: 1) definethe pathways mediating upregulation of TACE activity in primary human monocytes;2) compare the regulation of TACE catalytic activity and TACE substrate shedding;3) investigate the acute inflammatory response in elective surgical patients withparticular focus on three potential biomarkers of inflammation: TACE expression, HLA-DR expression and circulating monocyte subset populations. We used a fluorimetric assay of TACE activity to demonstrate thatlipopolysaccharide (LPS) stimulation of primary human monocytes promotes rapidupregulation of TACE activity, independently of changes in TACE expression. TACEactivity upregulation was mediated by reactive oxygen species-induced activation ofthe p38 MAPK-MK2 pathway. Dithiol oxidation was found to induce L-selectinshedding but to attenuate LPS-induced TACE activity upregulation. Investigation ofthis paradox revealed that L-selectin shedding may be independent of TACE activityupregulation. Monocyte TACE expression was not modified by low to intermediaterisk surgery. In contrast, surgery resulted in downregulation of monocyte HLA-DRexpression and differential trafficking of monocyte subsets. These data provideinsight into the regulation of TACE during acute inflammation.

The ability to proliferate independently of signals from other cell types is a fundamental characteristic of tumor cells. Using a 3D culture model of human breast cancer progression, we have delineated a protease-dependent autocrine loop which provides an oncogenic stimulus in the absence of proto-oncogene mutation. Inhibition of this protease, TACE/ADAM17, reverts the malignant phenotype by preventing mobilization of two crucial growth factors, Amphiregulin and TGF[alpha]. We show further that the efficacy of EGFR inhibitors is overcome by physiological levels of growth factors and that successful EGFR inhibition is dependent on reducing ligand bioavailability. Using existing patient outcome data, we demonstrate a strong correlation between TACE and TGF[alpha] expression in human breast cancers that is predictive of poor prognosis.
