Aim: To evaluate the efficacy of different types of rehabilitation with fixed or removable full-arch implant-supported prosthesis designs in terms of implant loss and success in patients with at least one edentulous jaw, with tooth loss mainly due to periodontitis. Materials and methods: Clinical studies with at least 12)months reporting on implant loss and implant success were searched. Meta-analysis was conducted to estimate cumulative implant loss considering different prostheses designs. Results: A total of 11 studies with unclear to low risk of bias were included in the analysis. Estimated cumulative implant loss for fixed prostheses within 1)year and 5)years was 0.64% (95% confidence interval [CI]: 0.31%-1.31%) and 1.85% (95% CI: 0.85%-3.95%), respectively. The corresponding values for removable prostheses amounted to 0.71% (95% CI: 0.22%-2.28%) and 4.45% (95% CI: 2.48%-7.85%). Peri-implantitis affected 10%-50% of the patients restored with implant-supported fixed prostheses. Conclusions: Based on the limited low-quality data, the present analysis points to a low and similar cumulative implant loss within 1 year for patients with tooth loss mainly due to stage IV periodontitis restored with either removable or fixed implant-supported full-arch prosthesis. At 5)years of functioning, there was a tendency for better outcomes using fixed designs.

Abstract: Background Long-term data (>10 years) concerning the survival and success rates of implants and implant-supported prostheses are scarce. Purpose The present investigation represents one of the first studies on dental implants covering an observational period of 25 years. Materials and methods This study presents the results obtained in 26 patients with 75 implants who participated over a 23- to 28-year period in a supportive implant therapy (SIT) program at a private dental practice. We extracted existing data from the patients' files (pocket depths [PDs], bleeding on probing [BoP], radiographic peri-implant bone loss, and survival rates of the implant-supported prostheses). Results After 25 years, the SIT-compliant patients' implants had a survival rate of 95% (prostheses: 88%). The mean peri-implant probing depth was 3.69 mm (median: 3.33; SD: 1.06; range: 2-8.33). The mean peri-implant bone level was 1.84 mm (median: 1.82; SD: 1.20; range: −0.97-5.2). Finally, the prevalence (moment of last consultation) and incidence (during the entire observational period) of peri-implantitis were 7% and 30%, respectively. Conclusions Under SIT conditions, clinicians may expect survival rates for implant-supported prostheses of >80%. Most implants (60%) did not develop signs of peri-implantitis over a 25-year period

Purpose: To evaluate whether 5-mm short dental implants could be an alternative to augmen- tation with anorganic bovine bone and placement of at least 10-mm long implants in posterior atrophic jaws.Materials and methods: Fifteen patients with bilateral atrophic mandibles (5 to 7 mm bone height above the mandibular canal) and 15 patients with bilateral atrophic maxillae (4 to 6 mm bone height below the maxillary sinus), and bone thickness of at least 8 mm, were randomised according to a split-mouth design to receive one to three 5-mm short implants or at least 10-mm long implants in augmented bone. Mandibles were vertically augmented with interpositional bone blocks and maxillary sinuses with particulated bone via a lateral window. Implants were placed after 4 months, submerged and loaded, after another 4 months, with provisional pros- theses. Four months later, definitive provisionally cemented prostheses were delivered. Outcome measures were: prosthesis and implant failures, any complication and peri-implant marginal bone level changes.Results: In five augmented mandibles the planned 10-mm long implants could not be placed and shorter implants (7.0 and 8.5 mm) had to be used instead. Five years after loading, six patients, five treated in the mandible and one in the maxilla, dropped out. Three prostheses (one mandibu- lar and two maxillary) failed in the short-implant group versus none in the long-implant group. In mandibles one long implant failed versus two short implants in one patient. In maxillae one long implant failed versus three short implants in two patients. There were no statistically signifi- cant differences in implant (n = 26; P = 1.00, difference = 3.85%, 95% CI: u221212.95% to 20.64%) and prosthetic (n = 26; P = 0.250, difference = 11.54%, 95% CI: u22120.74% to 23.82%) failures. Eleven patients had 16 complications at short implants (one patient accounted for six complica- tions) and 12 patients had 14 complications at long implants. There were no statistically signifi- cant differences in complications (n = 28; P = 1.00, difference = u22123.57%, 95% CI: u221230.65% to 23.51%). Five years after loading, patients with mandibular implants lost on average 1.72 mm at short implants and 2.10 mm at long implants of peri-implant marginal bone. This difference was statistically significant (difference = 0.37 u00b1 0.43 mm; 95% CI: 0.07 to 0.68 mm; P = 0.022). In maxillae, patients lost on average 1.31 mm at short implants and 1.79 mm at long implants. This difference was statistically significant (difference = 0.48 u00b1 0.43 mm; 95% CI: 0.22 to 0.74 mm; P = 0.002).

Abstract: The purpose of this study was to investigate the prosthetic complications of patients with a maxillary complete denture (CD) opposing a mandibular Implant- Supported Fixed/Removable Prosthesis (ISFRP). This study is a retrospective analysis of an ongoing prospective study. Dental records from 46 patients treated with a maxillary CD and a mandibular ISFRP were reviewed for fifteen different prosthetic complications. The mean recall time was 7.9 years. The percentage of patients exhibiting each complication and corresponding 95% confidence intervals were calculated. The recall period was divided into three parts: (A) 0-2 years (B) >25 years (C) 5 years. Logistic regression analysis determined the effect of recall period, age and gender on each of the following major complications: tooth fracture, complete denture relines, screw complications, and tooth replacement. Statistical significance was exhibited for: complete denture relines, posterior tooth replacement, and screw complications. The most common complications include prosthetic tooth fracture, tooth wear, maxillary hard relines, and screw complications. There were no incidents of framework fracture or abutment fracture. Patients were: .06 times more likely to require a heat processed hard reline with each year increase of age. Patients were 3.7 times after two to five years of service and 8.5 times after five years of service, more likely to require a hard reline than in the first two years of service. Patients were 52.5 times more likely to need posterior tooth replacement after five years of service than in the first two years, and 39.9 times more likely to encounter a screw complication after five years of service than in the first two years of service.
