Not illustrated. 1903 edition. Excerpt: ...Treatment. The treatment consists in complete rest in bed, the use of iron, arsenic, and strychnine, as well as transfusion. Amblyopia from Drugs. Some substances (tobacco, alcohol, etc.) produce amblyopia by means of an orbital optic neuritis, and the condition has been described under this heading. An amblyopia may arise, however, from the toxic influence of many other substances, such as nitrate of silver, mercury, nitrobenzol, cannabis indica, iodoform, chloral, lead, salicylic acid, and quinine. In quinine amblyopia the symptoms may appear after a moderate dose, although they are more frequently met with after excessive quantities have been taken. The symptoms of quinine-blindness are total loss of vision, extreme paleness of the optic disks, diminution of the retinal bloodvessels in number and calibre, and contraction of the field of vision. The result of an attack of quinine blindness may be partial or complete recovery of central vision. As demonstrated by de Schweinitz in his experiments upon dogs, the toxic influence of quinine is to lessen, through spasm of the vessels, the blood supply of the retina and optic nerve, and later permanent optic nerve atrophy may follow. It has been recently shown by Ward Holden that the effect of the diminished blood supply is a degeneration of the ganglion cells and nerve fibres of the retina, which is followed by an ascending degeneration of the optic nerve. Treatment. The treatment consists in inhalations of nitrite of amyl and the internal administration of strychnine and digitalis. The use of the drug should, of course, be discontinued.
