This publications argues that the existing user charges reveal a system lacking all logic. The cluster of systems perpetuates injustice, distorts medical priorities and hinders access to vital treatment. The Commission tries to bring greater coherence to the system, to help people on low incomes more effectively, to link charges to the value or benefit of the treatment or service, to encourage a preventative approach whenever possible, to better protect people with chronic conditions and to simplify the system exemptions.Some of the proposals include removing automatic exemption from prescription charges for over 60s and pregnant women, making a greater number of dental treatments subject to private provision and full cost pricing and limiting help with the cost of travel to and from hospital on the basis of income.

The Committee's report examines the use of healthcare charges within the NHS for certain treatments (such as prescriptions, dentistry and optical services) and non-clinical services (such as car parking fees and charges for television and telephone use), including the level of charges, categories of exemption, and whether the charging system is sufficiently transparent to users. It also considers whether such charges have a place within an NHS which claims to be free at the point of use, or whether these charges should be abolished and the necessary resources raised through alternative charging systems. International research indicates that health charges have a negative effect on health, particularly on patients with long-term illnesses. The report finds that the system of health charges in England is a mess and lacks any comprehensible underlying principles, developed largely through ad hoc responses to the need to raise money. The system of exemptions to charging is also full of anomalies, particularly in relation to prescription charges, and has not been updated since 1968 despite improvements in medical science. The Committee makes 22 conclusions and recommendations to improve the system, including the need for more research in England on public attitudes to health, on the effects of charges on patient health and the use of health services, and on the costs and benefits of alternative charging systems. The report recommends that a number of short-term changes be made immediately, including alterations to the prescription pre-payment certificate (PPC) to improve take-up levels, a review of new dental charges, free parking for patients who have to attend out-patient clinics on a daily basis and reduced rates for those who must attend hospital regularly.

The financial challenge facing the UK's health services over the next four years, combined with the relentless rise in the demand for services, means that it is imperative that the NHS delivers more for less. The authors of this report studied UK and international literature on hospital efficiency, and surveyed senior managers and clinicians in six hospital trusts that had been 'turned around' following financial difficulties. They found much research evidence on what measures are associated with higher productivity in hospitals. They gleaned practical lessons from the managers about what successful steps had been taken to improve efficiency in their financially challenged hospitals. An accompanying research summary highlights the report's key findings within the context of the current Government health reforms. Thanks to remarkable advances in modern health care attributable to science, engineering, and medicine, it is now possible to cure or manage illnesses that were long deemed untreatable. At the same time, however, the United States is facing the vexing challenge of a seemingly uncontrolled rise in the cost of health care. Total medical expenditures are rapidly approaching 20 percent of the gross domestic product and are crowding out other priorities of national importance. The use of increasingly expensive prescription drugs is a significant part of this problem, making the cost of biopharmaceuticals a serious national concern with broad political implications. Especially with the highly visible and very large price increases for prescription drugs that have occurred in recent years, finding a way to make prescription medicinesâ€"and health care at largeâ€"more affordable for everyone has become a socioeconomic imperative. Affordability is a complex function of factors, including not just the prices of the drugs themselves, but also the details of an individual's insurance coverage and the number of medical conditions that an individual or family confronts. Therefore, any solution to the affordability issue will require considering all of these factors together. The current high and increasing costs of prescription drugsâ€"coupled with the broader trends in overall health care costsâ€"is unsustainable to society as a whole. Making Medicines Affordable examines patient access to affordable and effective therapies, with emphasis on drug pricing, inflation in the cost of drugs, and insurance design. This report explores structural and policy factors influencing drug pricing, drug access programs, the emerging role of comparative effectiveness assessments in payment policies, changing finances of medical practice with regard to drug costs and reimbursement, and measures to prevent drug shortages and foster continued innovation in drug development. It makes recommendations for policy actions that could address drug price trends, improve patient access to affordable and effective treatments, and encourage innovations that address significant needs in health care.

Health systems need to set priorities fairly. In one way or another, part of this important task will fall to physicians. How do they make judgments about resource stewardship, and how should they do so? How can they make such decisions in a manner that is compatible with their clinical duties to patients? In this book, philosophers, bioethicists, physicians, lawyers and health policy experts make the case that priority setting and rationing contribute significantly to the possibility of affordable and fair healthcare and that clinicians play an indispensable role in that process. The book depicts the results of a survey of European physicians about their experiences with rationing and other cost containment strategies, and their perception of scarcity and fairness in their health care systems. The National Health Service spends £8 billion a year on prescription drugs in primary care in England. This has grown 60% in real terms over the last decade and the trend continues to be upward. This report examines how the money could be spent more efficiently, without compromising clinical outcomes. It looks principally at the scope for more cost-effective prescribing; supporting the GPs and PCTs in getting better value for money; and the wastage of drugs. For four groups of drugs (which account for 19% of the total drugs bill), it finds a wide disparity in prescribing behaviour. It estimates that if all PCTs attained the standard of the best 25%, £200 million could be saved. The analysis shows that there are several mechanism to improve value for money. They include: personalised communication with GPs from local experts, financial and practical incentives, and involving the whole primary and local care community in decisions about drug policy. Wastage of drugs is a problem but the level is not monitored and the uptake of initiatives to reduce the amount of waste is low. Assessing whether local prescribing volumes are consistent with clinical need is complex, but combining prescription data with local prevalence data can provide benchmark information.

The Health Committee is highly critical of the delay in setting out precisely what a value-based pricing system for drugs entails. There is also uncertainty about the implications of the changes proposed for the Cancer Drugs Fund which was introduced in 2011 to allow clinicians to use drugs that had not been approved by NICE, and which will be superseded by the value-based pricing system. The Committee calls for: an assessment of the outcomes for those patients whose treatment has been paid for by the Cancer Drugs Fund; evidence of beneficial outcomes which should inform the new value-based pricing scheme and applied to treatments of conditions other than cancer; and clarity about how drugs which have been paid for by the Fund will continue to be available to individual patients. There is also concern about the implications for the effectiveness of NICE of recent evidence about access to information from clinical drug trials. There should be both a professional and legal obligation to ensure that all regulators, including NICE, have access to all available research data about the efficacy and safety of pharmaceutical products which are in use in the UK. The pharmaceutical industry should introduce a new code of practice to make this commitment effective and the GMC should reiterate its guidance to doctors on the conduct of drug trials. Is important for the credibility of NICE that Patient voice is effectively and openly represented in all its work; and that NICE guidance should continue to be guidance rather than an instruction and that the NHS should continue to allow local discretion, but variations from NICE guidance should be open, transparent and accountable
