In the last decade, the incidence of acute poisoning in the developed world has risen considerably, so that hospital admissions from this cause now repre~ sent a significant part of the work load of most medical units. The purpose of this book is to provide an up-to-date account of the diag nosis and treatment of all the clinically important poisons. Throughout the text the aim has been to emphasize the mechanisms of toxicity - wherever they are known - so that a rational approach to therapy may be devised. In addition, substantial chapters have been devoted to the psychiatric assess ment of self-poisoned patients and to the role of the laboratory. Two styles of presentation have been adopted. Certain topics are discussed in depth either because of their clinical importance or because recent data have become available concerning pathophysiology or treatment. In contrast, when subjects have been well described previously in general medical texts, for example, carbon monoxide poisoning, the discussion is far more brief and to the point. A similar style has been adopted with recently introduced drugs, where little is known about the effects in overdose. The value of cer tain chapters, especially those on poisonous plants and snake bites, has been enhanced by the inclusion of a large number of colour photographs.

The aim of this book is to present the current state of the art of extracting natural products with near-critical solvents and to view the possibilities of further extensions of the technique. Relevant background theory is given but does not dominate the book. Carbon dioxide is the near-critical solvent used in most recent applications and inevitably receives prominence. In addition to general descriptions and reviews, the book contains three chapters by indus trial practitioners who describe in detail the operation of their processes and discuss the market for their products. Sections on the design of the pressure vessels and pumps required in these processes and on the acquisition of the data required for design are included. The costing of the processes is also discussed. There is good scope for combining a near-critical extraction step with other process steps in which the properties of near-critical solvents are utilised, for example as a reaction or crystallisation medium and a chapter is devoted to these important aspects. It is hoped that the work will be found to contain a great deal of specific information of use to those already familiar with this field. However the style of presentation and content is such that it will also be useful as an introduction. In particular it will be helpful to those wondering if this form of separation method has anything to offer for them, whether they are engineers, chemists or managers in industry, or in academic or research institutions.

This treatise had its origins in the authors' strong opinion that the discovery of new drugs, especially of innovative therapeutic agents, really does not happen as a spontaneous sequel to investiga tive research, no matter how penetrating such research may be. Rather, it seemed to us that the discovery of innovative therapeutic agents was a very active process, existing in and of itself, and demanding full attention-it was not simply a passive, dependent by-process of investigative research. And yet, many researchers some close confreres of the authors, others more distant-believed otherwise. We felt that their view reflected unrealistic thinking and that reality probably lay closer to what Beyer" maintained: We are taught to believe that if we can understand a disease it should be easy enough to figure out, say, the molecular configuration of a definitive receptor mechanism somewhere along the line and to design a specific drug . . . . And so we start out to understand the disease but never get around to doing much about therapy. The authors very soon realized that there was essentially no quantitive information available on just where and how innovative therapeutic agents were discovered. There were only anecdotal accounts, and these were able to be selected and presented in ways that could be used to defend any point of view.

Many chemotherapeutic agents introduced for use in humans are carcinogenic in laboratory animals (Conklin et al. 1965; Shimkin et al. 1966; Griswold et al. 1968; Harris 1976). However, initially their beneficial effect in disseminated cancer was of such short duration that the inevitable death of the patient from his primary disease precluded any clinical manifestation of the carcinogenic potential. During the last decade, chemotherapy has radically changed the outlook for many patients with cancer. Combinations of drugs, administered as the primary treatment, have resulted in high rates of cure in patients with disseminated malignancies, such as stage IV Hodgkin's disease or childhood acute lymphocytic leukemia. In other disseminated forms of neoplasia, induction of a remission, a substantial palliation and a prolongation of survival have been achieved. In many instances of localised disease, where surgery with or without radiotherapy are the primary form of treatment, anticancer drugs have been used with success as adjuvant therapy for distant microscopic disease. With these spectacular achievements, secondary malignancies, in particular acute non-lymphocytic leukemia (ANLL), has become of major concern. Incidence Acute leukemia is the most frequent form of secondary neoplasia in patients treated for cancer (Penn 1981). In one large series, 5. 9% of all ANLL could be attributed to previous chemotherapy (Kapadia et al. 1980).
