Insight is a multidimensional construct, defined as the awareness of having a mental disorder, of specific symptoms and their attribution to the disorder, the awareness of social consequences, and of need for treatment. Although insight has been studied specifically in schizophrenia and its study in mood disorders has traditionally received limited attention, the evaluation of this concept in mood disorders is also very important because of the impact on treatment compliance and outcome. In bipolar disorder (BD), clinical insight varies substantially over time. Most researchers observed that insight is more impaired during an illness episode than during remission, in mixed than in pure manic episodes, in bipolar II than in bipolar I patients, and in pure mania than in bipolar or unipolar depression. Lack of insight is a consistent factor of non-adherence to medication in bipolar patients, along with severity of BD, side effects of medication, effectiveness, and patient-related factors. Also, impaired insight into treatment and a great number of previous hospitalizations are associated with poorer clinical outcomes (psychiatric hospitalization, emergency room visits, violent or suicidal behavior) among the patients with bipolar I disorder. In the management of bipolar disorder, improving quality of life (QoL) and outcome should be one the most important goals.

Objectives: The objectives of the study are to evaluate the relationship between insight and psychotic symptoms in first-episode psychosis (FEP) and to explore the relationship of insight and social aspects. Background and aims: The lack of insight has been related to symptom intensity (Nieto, 2012) and to the duration of untreated psychosis (DUP) in FEP (Cuesta, 2011). Only a few studies link the insight to social components, suggesting a correlation with the level of education (Elowe and Conus, 2017). Materials and methods: Sample consisted of 91 patients with a FEP. Inclusion criteria: minimum 2 psychotic symptoms, age between 7-65 years, maximum 6 months from initial contact with medical services. Instruments: PANSS, sociodemographic questionnaire, DAS (Disability Assessment Schedule), LSP (Life Skills Profile). SUMD to evaluate lack of insight (SUMD1: awareness of mental disorder, SUMD2: awareness of the effects of medication, SUMD3: awareness of the social consequences). Pearson's correlation was used for the analysis. Results: There was significant correlation between all the SUMD and PANSS subscales. No significant relationship was found between DUP and insight. Years of education correlated with SUMD3 (p =, 026). SUMD1 and SUMD3 correlated with DAS: Family and social disability (p =,012; p =,004), while SUMD2 correlated with the LSP: Self-care (p =,012). Conclusions: The relationship between insight and symptom intensity is replicated, but not the correlation with the DUP. There is a link between insight and certain psycho-social variables, different relationships are perceived depending on the dimension of insight.
