Forty brain damaged mental hospital patients and forty non-organic psychiatric patients matched on age, sex, and length of hospitalization were tested with the Archimedes spiral following pretraining with stimulus situations which involved actual rather than illusory expansion and contraction. Subgroups of each type of patient were examined under immediate and delayed response conditions following fixation of the Archimedes spiral on a series oftest trials. Estimates of the duration of the spiral aftereffect (SAE) were taken, along with measures of response latencies and continuous GSR records. The results confirmed the expectation that brain damaged Ss under immediate response conditions would report perception of the SAE as frequently as a control group of comparable non-organic psychiatric patients. Fifteen of the twenty brain damaged Ss under immediate response conditions reported the SAE on six or more of the eight test trials. All of the brain damaged Ss in this condition reported the SAE on at least one of the test trials. The results also supported the prediction that Ss under delayed response conditions would give significantly fewer responses indicating perception of the SAE than Ss under immediate response conditions, Neither age, length of hospitalization, nor visual acuity significantly differentiated any of the subgroups in terms of frequency of SAE report. No significant differences were found between the organic subgroups and the corresponding control groups in frequency of SAE report on successive test trials. None of the subgroups differed significantly in terms of the duration of the SAE.

This investigation concerns itself with the validation of previous studies and the investigation of some heretofore untapped parameters of the phenomenon, e.g., extinction of the response, speed of rotation, and difference, if any, in difficulty between the two spirals. Six proposals were explored: 1. that normal subjects and non-organic psychiatric patients will report the distinctive spiral visual aftereffect, while brain-damaged patients will not report it or will do so to an incomplete degree; 2. that there will be no relationship between age or intelligence and the reported experience of the spiral visual aftereffect; 3. that brain-damaged patients who report the aftereffect will show a significantly longer duration of the phenomenon than will normals or non-organic psychiatric patients; 4. that brain-damaged subjects will require fewer trials to subjects; 5. that there will be no relationship between speed of rotation of the spirals and the report of the aftereffect; and 6. that subjects will experience the aftereffect of spiral A with greater ease than they will experience the aftereffect of spiral B. The sample consisted of 15 organics, 15 non-organic psychiatric patients, and 15 normals, drawn from patients and staff of the Psychiatric Receiving Center of Kansas City, Missouri. Prior to testing with the Archimedes Spiral, each subject was administered the full scale Wechsler-Bellevue Intelligence Scale, Form I. The procedure and instructions for the (SVA) Spiral Visual Aftereffect Test were essentially the same as those described in detail by Price and Deabler. It was found: 1. that organic patients report significantly lower scores on the SVA than do non-organic psychiatric patients or normals; 2. that there was no relationship between age or intelligence and the reported experience of the SVA; 3. that the groups did not differ in length or duration of the aftereffect response; 4. that the groups did not differ in number of trials to extinction of the afereffect response; 5. that there was a trend for lower scores to occur at the lower rotation speed of 25 r.p.m., as opposed to 100 r.p.m., but this trend was not significant; and 6. that subjects were able to perceive the aftereffect of spiral A with greate ease than they were able to perceive the aftereffect of spiral B. While the results, in general, agree with those of Price and Deabler it was found, in concurrence with Spivack and Levine and Page, et al., that the discriminatory power of the SVA test is considerably poorer than reported by Price and Deabler. Although the instrument appears to be sensitive to organic brain damage, the high rate of misclassification lends doubt as to its clinical usability as an effective instrument for the diagnosis of brain damage. When the base rate technique of Stilson, et al., was employed, it was found that the SVA test results in a higher percentage of misdiagnoses than would occur if no test had been used at all and all patients in the hospital population had been classified non-organic. The trend for lower scores to occur at lower rotation speed and the significant findings, in regard to the difference in difficulty between the two spirals, suggest that further research along these lines might prove fruitful as a source of reducing false positives and negatives and thereby increasing the clinical utility of the instrument.
