Many Gulf War veterans have reported an array of physical and mental health complaints since the war and the question on everyone's mind is Can we identify a cause or is this a coincidence? To complement efforts by the Defense Department and other federal agencies in their attempt to understand the full range of health implications of service in that conflict, RAND undertook a major study to summarize the scientific literature on the known health effects of given exposures to such risk factors as oil well fires, chemical and biological warfare agents, depleted uranium pesticides, pyridostigmine bromide, immunizations, infectious diseases, and stress. In this landmark book series the authors report on their literature review summarize what is known about the range of actual exposures in the Gulf, and assess the plausibility of the risk factor at hand as a cause of illness. The authors caution that more research on health effects and exposures remains to be completed before definitive statements can be made and make recommendations for additional research.

Gulf War and Health, Volume 2, is the second in a series of congressionally-mandated studies by the Institute of Medicine that provides a comprehensive assessment of the available scientific literature on potential health effects of exposure to certain biological, chemical, and environmental agents associated with the Gulf War. In this second study, the committee evaluated the published, peer-reviewed literature on exposure to insecticides and solvents thought to have been present during the 1990-1991 war. Because little information exists on actual exposure levels â€" a critical factor when assessing health effects â€" the committee could not draw specific conclusions about the health problems of Gulf War veterans. However, the study found some evidence, although usually limited, to link specific long-term health outcomes with exposure to certain insecticides and solvents. The next phase of the series will examine the literature on potential health effects associated with exposure to selected environmental pollutants and particulates, such as oil-well fires and jet fuels.

The Gulf War in 1990-1991 was considered a brief and successful military operation, with few injuries or deaths of US troops. The war began in August 1990, and the last US ground troops returned home by June 1991. Although most Gulf War veterans resumed their normal activities, many soon began reporting a variety of nonexplained health problems that they attributed to their participation in the Gulf War, including chronic fatigue, muscle and joint pain, loss of concentration, forgetfulness, headache, and rash. Because of concerns about the veterans' health problems, the Department of Veterans Affairs (VA) requested that the Institute of Medicine (IOM) review the scientific and medical literature on the long-term adverse health effects of agents to which the Gulf War veterans may have been exposed. This report is a broad overview of the toxicology of sarin and cyclosarin. It assesses the biologic plausibility with respect to the compounds in question and health effects.

Mr. Chairman and distinguished Members of the Sub-Committees, it is a pleasure for us to address you today on RAND's review of the scientific literature as it pertains to pyridostigmine bromide (PB) and illnesses among Gulf War veterans. RAND was asked to perform a literature review to evaluate whether PB could plausibly be related to increased health symptoms experienced by Persian Gulf War (PGW) veterans. I examined over 10,000 titles, 6,000 abstracts, several thousand papers and reports, interviewed over 80 people, and reviewed dozens of declassified British studies and reports.

The United States and its allies in the Persian Gulf War (PGW) knew that Iraq had used nerve agents and chemical weapons in its previous conflicts and so took steps to protect their troops. Pyridostigmine bromide (PB) was distributed as a pretreatment that would enhance the effectiveness of postexposure treatments in the event that the nerve agent soman was used. This report examines the role that PB played in the ongoing chronic health problems documented in PGW veterans. After careful examination of the known effects of PB on the central and peripheral nervous systems, the author finds the evidence consistent with a possible role for PB as a contributor to the health complaints of PGW veterans and calls for immediate attention in the form of additional investigation to clarify the role of PB.
