A two-month-old Ayrshire heifer calf was presented to Cornell's Equine and Nemo Farm Animal Hospital on August 12, 2017 for bilateral abdominal distention, decreased manure production, anorexia, and depressed attitude. The patient had been weaned from bottle feeding to pail drinking five days prior to presentation, and was otherwise healthy at weaning. Physical examination confirmed the bilateral abdominal distention and also revealed melena and a fluid-filled viscus in the right caudoventral abdomen, which was identified as the omasum on ultrasound examination. Based on the abnormal position of the omasum, a presumptive diagnosis of right-sided abomasal displacement with volvulus was made, and an emergency exploratory celiotomy was performed via the right paralumbar fossa. Surgical exploration confirmed a right-sided displacement, counterclockwise volvulus of the abomasum, and secondary caudolateral displacement of the omasum. The gas-distended abomasum was decompressed and assessed for tissue viability before being replaced in its proper orientation; no abomaso-, pyloro-, or omentopexies were performed. The patient recovered uneventfully from anesthesia and appeared clinically normal within one hour of surgery. This patient's case was unique because, while displacement of the abomasum can technically occur at any age or stage of lactation, it is typically seen in lactating dairy cattle in the first four-to-six weeks of the post-partum period. Abomasal displacement and volvulus in calves has rarely been reported, whereas there is a plethora of information on the disease process in adult cattle. This report will work through this case as well as provide an overview of the epidemiology of abomasal displacement and volvulus in calves and adult dairy cattle.
