For most people today, ABO-incompatible kidney transplantation still carries the image of pronounced hyperacute rejection. However, with modern immunosuppressive therapy, hyperacute rejection will not occur within the first 24 hours. Clinical statistics in Japan show 3 cases of hyperacute rejection, but all of these occurred because the recipient was mistakenly transfused during surgery with frozen plasma of the same blood type. Delayed hyperacute rejection, occurring after the first 24 hours, is most likely to develop within 1 week post transplant. It does not manifest suddenly, but several years after transplantation. This means that humoral rejection ceases to occur after accommodation has been established. The use of evidence-based medicine to correct misconceptions in this area has proven to be meaningful. The publication of these data has made it possible to break down fixed thinking and prejudices previously regarded as fact in relation to ABO-incompatible kidney transplantation. It is believed that this will prove to be a major contribution in the future development of immunology. As these data are provided as a common resource for physicians performing transplantation procedures, it has encouraged more widespread implementation of organ transplantation.

ABO incompatible kidney transplantation is indicated for patients for whom no ABO-identical or minor mismatch donor is available. Since the author and his colleagues performed the first ABO-incompatible kidney transplantation in Japan in 1989, 400 such transplantations have been performed in 41 hospitals in Japan and this practice has contributed to a number of new developments. One is a clearer and more suitable model for conceptualising the mechanism of humoral immune response which enables identification of antigens and antibodies and a therapeutic strategy against rejection. The rejection mechanisms are discussed not only from the perspective of immunology but also viewed from different angles, including anatomy, microscopic and macroscopic pathology, molecular biology and haematology. Immunosuppressive therapy is discussed, divided in four categories: extracorporeal immunomodulation with removal of humoral antibodies; drug therapy to suppress cellular immunity; splenectomy; and anticoagulation therapy. Surgical procedures for kidney transplantation and splenectomy are treated, including discussion of the best timing for the latter. The book gives an overview of the current status with statistics and results of questionnaires and ends with discussions of 17 case histories.

Following the great success of Accommodation in ABO-incompatible Kidney Transplantation published in September 2004, Elsevier is proud to present the publication of ABO-incompatible Organ Transplantation from Japan, also edited by Professor Kota Takahashi. This new publication consists of a collection of invited papers from the International Meeting at the 41st Annual Meeting of the Japan Society for Transplantation held in Niigata on 29 October 2005. Since the donation of cadaver organs is extremely rare in Japan, ironically, ABO-incompatible transplants and partial liver transplants from living donors have become prevalent as a way of expanding the accommodation of transplants. More than 700 ABO-incompatible kidney transplants and over 300 ABO- incompatible liver transplants have been performed in Japan, and the results improve yearly.

Introduction: Recipients of an ABO incompatible transplant have circulating anti-AB antibodies, A and/or B antigens expressed on endothelium and usually C4d deposition on the endothelium, yet with adequate immunosuppressive treatment, this seldom leads to antibody-mediated rejection. Previous work using RNA sequencing compared ABO-incompatible (ABOi) and ABO-compatible (ABOc) surveillance biopsies with normal histology and identified a number of differentially expressed genes. Here we validate CYP4F11 as a marker of accommodation in an independent set of biopsies.Methods: RNA was obtained from formalin fixed paraffin embedded tissue from 20 ABOc and 9 ABOi renal transplant patients. Eighteen genes considered the top hits in sequencing analysis were assayed using a Nanostring nCounter custom panel. Results: Comparison between two groups confirmed a reduced expression of CYP4F11 in ABOi patients compared to ABOc (p=0.0005, Mann-Whitney test) (See figure). None of the other genes demonstrated a significant differential expression between the two groups. Discussion:Expression of CYP4F11 is reduced in ABOi patients confirming association with accommodation of the renal allograft. The study may be under-powered to validate any of the other genes. Most of the substrates of cytochrome p450 4F isoforms are eicosanoids, which play important roles in the inflammatory response. Cytochrome 4F enzymes metabolize arachidonic acid to 20-hydroxyeicosatetraenoic acid (20-HETE). CYP4F11 is known to have high expression in the kidney and 20-HETE inhibitors have demonstrated improved renal microvascular function in hypertension. The down-regulation of CYP4F11 in ABOi transplants may be likewise protecting the microvasculature.
