Excerpt from Anaesthetics: Their Uses and Administration Since the issue of the earlier editions of this book a material advance has been made in our knowledge of anaesthesia and analgesia; while the older methods have become more accurate many new and valuable ones have been added to the anaesthetists armamentarium. This has necessitated rewriting most of the old sections, deletion of obsolete apparatus and theories, and a considerable addition of new material. As illustrations render the letterpress easier of comprehension, especially when apparatus are concerned, a large number of photographs and woodcuts have been added. Eight full-sized plates have been inserted, some of which point out the position to be adopted by the patient and anaesthetist during certain methods, while others indicate the course and surface-marking of the direction of the principal sensory nerves and arm veins, thus facilitating the carrying out of regional analgesia and intra-venous infusion. The occurrence of the International Medical Congress in 1913 and the focussing of modern ideas on analgesia and anaesthesia in the subsection devoted to these subjects enabled me to obtain at first hand from the actual promoters of the more modern procedures the fullest and most reliable information. That it may be a guide to the student in the selec tion of an anaesthetic for his future patients is the hope of the author, and if it is also found of value as a work of reference for the busy practitioner, and thus ameliorates pain in the animal world, the time devoted to the deductions and opinions brought for ward - many of them quite contrary to the usual teaching - will not have been spent in vain. To reason for the animal world merely from analo gies with human patients is liable to give rise to grave errors, as indeed is also the attempt to reason that because one species of animal is a good subject for a certain anaesthetic every other kind must be. Excerpt from Anaesthetics and Their Administration: A Text-Book for Medical and Dental Practitioners and Students The fourth edition of this text-book was edited by myself under the immediate supervision of the late Sir F. W. Hewitt, its original author. The responsibility for the statements and opinions contained in that edition was his. Since then the advance of the science of anaesthesia has been more rapid than in any similar period of time, except, perhaps, the years following its discovery. In consequence, large sections of this work have been entirely rewritten, and there is scarcely a page which has been allowed to stand unrevised. Sir Frederic Hewitt's breadth of mind and keenness for progress were so conspicuous that he would have been among the first to realise that much of the teaching current a decade ago has been rendered obsolete by recent developments. I hope that the opinions expressed in this edition would have commended themselves to him, if he had survived; right or wrong, however, the entire responsibility for the clinical teaching in this edition is solely mine, not his. As in the fourth edition, Mr. Digby Cotes-Preedy has been kind enough to be responsible for the chapter on the Medico-Legal Aspects of Surgical Anaesthesia, for which my best thanks are due to him. For the revision of the two chapters dealing with the Physiology of Anaesthesia I succeeded in enlisting the co-operation of Dr. A. J. Clark, Professor of Pharmacology at University College, London. The masterly synopsis of this difficult subject which he has produced makes me profoundly thankful that I did not attempt the task myself, for I am sure that I could not have carried it out with anything approaching the same success. Excerpt from Chloroform, the Best of Anaesthetics During the inhalation I watch the face more than I do the pulse, although the face, the chest movements andwthe pulse, as felt in the temple, are all kept under constant observation. The color and expression of the face will always be a sure index of the working of the heart and lungs. -as soon as sleep begins to show itself, I take away the pillow from the head of the patient, so as to insure a horizontal decubitus, which would often be seriously. Objected to by the patient during the sensible condition. Should symptoms of vomiting show themselves, the patient is immediately turned over on the side until emesis is accomplished. The anaes thetic is continued, notwithstanding the retching. I find the continued inhalation of the anaesthetic the best method for checking that involuntary contraction of the abdominal muscles, which accompanies vomiting.
