L’étude de la trajectoire des patients en cancérologie est un enjeu majeur tant pour la planification sanitaire que pour évaluer l’observance des recommandations de bonnes pratiques des professionnels de santé. Le cancer du sein représente le premier cancer féminin en termes d’incidence et de mortalité en France. Sa prise en charge fait l’objet de recommandations nationales de bonnes pratiques, coproduites par l’Institut National du CAncer et la Haute Autorité de Santé. L’objectif de cette étude est de définir une méthode d’analyse des trajectoires des patients en cancérologie à partir des bases médico-économiques, en prenant pour exemple la trajectoire de soins des patientes atteintes de cancer du sein en Haute-Normandie. Méthodes Les données sont issues d’une part des bases du programme de médicalisation des systèmes d’information (PMSI) pour l’hospitalisation de médecine – chirurgie - obstétrique de 2010 à 2013 ; d’autre part des données issues de la base SNIIRAM pour les données de radiothérapie libérale, des molécules onéreuses et de la détection pré-opératoire du ganglion sentinelle réalisée hors hospitalisation. L’étude a porté sur les patientes résidant en Haute-Normandie ainsi que les patientes prises en charge partiellement ou totalement en Haute-Normandie. Ont été incluses les patientes ayant un séjour en lien avec le traitement du cancer du sein en 2012 mais n’ayant pas de tel séjour en 2010 ni 2011. Les prises en charge de chimiothérapie, chirurgie et radiothérapie ont permis de décrire la trajectoire des patientes depuis leur première hospitalisation jusqu’à fin 2013 par l’étude des flux des patientes, des coopérations inter-établissements et du parcours de soins des patientes. Résultats 1873 patientes ont été incluses. Les établissements du territoire de santé (TS) de Rouen-Elbeuf ont une forte attractivité sur l’ensemble des patientes de Haute-Normandie. Les flux des patientes vers le site régional de référence en cancérologie, le CRLCC Henri Becquerel prédomine. Le TS de Dieppe se caractérise par un taux importante de fuites vers les autres TS haut-normands, les fuites extra régionales concernent principalement le TS d’Evreux-Vernon. Dans 81% des cas les patientes ont été prises en charge par un seul établissement. Les principales coopérations inter-établissement associent les cliniques privées aux centres de radiologie privés. 71% des coopérations se font entre des établissements ayant le même statut et 64% entre des établissements du même territoire de santé. 81,9% des patientes suivaient une prise en charge conforme aux recommandations. Conclusion Les données de la radiothérapie libérale ont permis la description précise des trajectoires des patientes atteintes d’un cancer du sein en Haute-Normandie. Les trajectoires pourront être approfondies par l’analyse de l’ensemble des séjours prenant en charge le cancer du sein. Les trajectoires de soins en cancérologie pourront être décrites à un niveau national en incluant la radiothérapie privée.

This book is a compilation and discussion of data on the survival of cancer patients in 12 European countries. Measures of incidence, survival and mortality are critical to the interpretation of data on progress in the fight against cancer, and in the evaluation of the overall effectiveness of cancer control programmes. Randomized controlled clinical trials have shown many modern protocols for cancer treatment to be more effective than earlier treatments, but until now, comparable population-based survival figures have raraly been available. EUROCARE is a concerted action among European cancer registries, aimed at estimating and comparing the survival of cancer patients in different European populations. The rationale behind this project is to optimize the comparability of survival data by using an agreed and standard definition of the diseases for which survival is to be compared, and by taking due account of basic demographic variables. The EUROCARE Working Group succeeded in collecting, checking and editing data on cancer survival from 30 cancer registries in 12 countries, and established a database covering some 800,000 cancer patients in the period 1978-85 and followed up to the end of 1990. This database forms the raw material for this monograph.
