Hypertension has certainly been one of the topics most fre quently discussed at symposia, meetings, and congresses during recent years. There may be several reasons for this; three of them are obvious: firstly, the fact that a large proportion of the world's population is suffering from various forms of hypertensive disease; secondly, increasing knowledge of the pathogenesis of hypertension and of the disturbances underlying it; and, thirdly, the marked progress which has been made in antihypertensive therapy over the past fifteen years. When plans for the present symposium were being drawn up, it was felt that it should not simply bring forth just another meeting on hypertension, but should place particular emphasis on those aspects which had not been adequately discussed at previous symposia of this kind. Curiously enough, the topic which appeared to have received least attention in the past was therapy, although from the practical point of view this is by far the most important. The choice of therapy as the main theme of the whole symposium also seemed to be warranted in view of the relatively long period that had elapsed since effective antihyper tensive treatment became available; the time had in fact come now to pass judgement on the benefits as well as the shortcomings of drug treatment as available today.

The previous volume on Antihypertensive Agents in the Handbook of Experi mental Pharmacology, published in 1977, was edited by the late Franz Gross from the Department of Pharmacology in Heidelberg, who was one of the grand old men in hypertension research. Now, more than 10 years later, it is necessary to update this volume. From the early days of antihypertensive drug treatment, starting about 30 years ago with drugs such as reserpine and guanethidine, the pharmacology of cardiovas" cular therapy has evolved into a highly sophisticated and effective therapeutic regimen. The major breakthroughs in the 1960s were the introduction of diuretics and beta-blockers. Then, in the 1980s, came the calcium antagonists and con verting enzyme inhibitors. It can be anticipated that the next decade will see a further expansion and sophistication of blood pressure lowering drugs. This book provides a state-of-the-art discussion of chemical, experimental, and clinical pharmacological data as well as of practical experience with drugs which are presently being used or which are going to be introduced on the market in the near future. The purpose of this volume is to provide a complete discussion of antihypertensive agents. Each major class of antihypertensive drugs is treated exhaustively in a separate chapter, fully referenced with chemical formulae, and richly illustrated with figures and tables. International authorities were asked to contribute in their respective fields of expertise.

This book brings together papers presented at an international symposium on centrally acting antihypertensive agents held in Geneva, Switzerland, in association with the 10th Scientific Meeting of the International Society of Hypertension. A major focus of this symposium was the sympatholytic agent, clonidine, and was partly stimulated by the re cent development of an innovative transdermal system for administering this antihyper tensive drug. Although clonidine has been available to clinicians for several years, there has been a re cent reawakening of interest in this type of medication. The centrally-acting antihyper tensive agents appear to be effective both as monotherapy and in combination with other drugs. There are no significant contraindications to their use, and they do not appear to produce metabolic side effects. In this symposium we have paid attention to two types of patients: those with uncomplicated mild hypertension, and those with more difficult forms of hypertension associated with concurrent conditions.

Michael Weber, MD, and a panel of distinguished authorities provide a comprehensive practical survey focusing on the key issues that determine the who, when, and how of hypertension management. Emphasis is given to the practical issues of management: how to select treatments that optimize results for even the most difficult-to-treat hypertensive patient, and what to do about the major concomitant problems. Examined in detail are the principal underlying mechanisms of hypertension, particularly those that help explain treatment choices, as well as the best techniques for successfully evaluating patients before treatment. Authoritative and eminently readable, Hypertension Medicine constitutes a highly practical account of the challenging issues faced daily by family physicians, internists, and general practitioners in the evaluation and optimal management of hypertension.
