Excerpt from Archives of Medicine (London) The following case illustrates many of the clinical points usually observed in cases of this affection. The spleen was of very large size, and the white corpuscles in the blood very numerous. History. - William Knight, aged 39, was admitted into the Tunbridge Wells Infirmary, January 11, 1859. He has for many years worked laboriously as a brickmaker and potter, but enjoyed, through life, excellent health, until three months ago, when he began to feel weak and unable to work, and he then noticed that his belly was getting very large and hard. Has had prolapsus ani and right inguinal hernia for several years. Had not lived in marshy districts, and never had ague. Present state. - Stature rather short, hair dark, aspect bilious but not cachectic, conjunctiva vascular but not jaundiced. He complains of anorexia, indigestion, diarrh a, abdominal distension, and disturbed unrefreshing sleep; can lie in any position, and with the head low. The abdomen is prominent, hard, and tense, measuring over thirty-six inches in girth, a hand breadth above the navel, and thirty-five inches round the navel. The liver is enlarged and somewhat protuberant, and descends to three inches below the margin of the ribs. Ogle, m.a., M.D., Cantab., Physician to the Derby Infirmary. T is generally acknowledged that clinical records are very unsatisfactory. They are either too long or too short too diffuse or too curt too particular or too indefinite too opinion ative or not sufficiently attested or, more commonly, they have all these faults in combination. They are diffuse and opinion ative on some points, even egotistical, asgae ad nauseam; and therein they are too long and too particular; at the same time, they are too general, or altogether silent on other matters, and this makes them lifeless, and practically too short. These defects are easily seen when the work is done one's own note book is full of them the difficulty is to avoid them ab initio. Each fault may be traced more or less directly to a common origin - the want of method. This paper therefore, is, in the main, directed to the advocacy of Method in Clinical Medicine. Diphtheria is chosen simply as an opportune illustration. The first step towards the cure of a disease is to understand its nature and essence; we care not so much for its name, as for its nature and its nature cannot be fully understood, with out searching into its cause or essence. In the same way, when we wish to remedy any other evil, such as that which is before us, we begin by making a plain statement of existing defects and, having done this, proceed at once to inquire into their origin. Excerpt from Archives of Medicine, 1859, Vol. 1: A Record of Practical Observations, and Anatomical and Chemical Researches Connected With the Investigation and Treatment of Disease Again the sound being used produced in the patient a sensation as if the instrument had struck against a ball at the top of the bladder. This was followed by a fluttering within her, as if something was moving. The constitutional derangement continued to increase, the local symptoms all became aggravated, and the repetition of the use of the sound was followed on several occasions by exceedingly violent convulsions. In order to prevent mischief from over distension of the bladder, the catheter was, on one occasion, left in the urethra. After passing a very restless night the sensation of motion became very distressing, and, although the urine had escaped as fast as it was secreted, the bladder seemed greatly enlarged. Mr. Barnett was now much surprised on removing the catheter to find, insinuated through its orifice, what appeared to him a roundish worm about the thickness of a piece of bobbin, an inch and a half in length, and of a white colour. Upon a repetition of this experiment, of which Mr. Lawrence also was now a witness, three worms were brought away, two of them most curiously entangled in the orifice of the instrument, and the third coiled round the end.
