La psiconcologia � una disciplina a cui afferiscono le culture di tipo psichiatrico, psicologico clinico, l''oncologia medica, la chirurgia oncologica, la radio e la chemioterapia, la terapia del dolore e riabilitativa. Nata negli Stati Uniti, � stata introdotta in Europa negli anni ''70 grazie al contributo fondamentale della psichiatra Elisabeth Kubler-Ross, che ha approfondito l''analisi delle reazioni e delle variabili connesse alla diagnosi di cancro. L''intervento psiconcologico � rappresentato dal sostegno offerto al paziente, nel corso del cammino terapeutico, individuale o di gruppo, e consente una rielaborazione dell''evento e un miglioramento dell''atteggiamento psichico verso la malattia. E'' una presa in carico globale del paziente affetto da neoplasia, che va dalla valutazione psicologica prima della comunicazione della diagnosi, alla fase riabilitativa e alla gestione psico-fisica del malato terminale. Lo psiconcologo ha il delicato compito di aiutare il malato con i suoi dubbi, le sue paure, i suoi conflitti, ma anche l''intero nucleo familiare.Questa area d''intervento include l''analisi delle tecniche comunicative che possono risultare utili a migliorare le variabili psicologiche rispetto alla compliance degli individui, la resilienza familiare e gli stili di coping. L''attenzione � rivolta agli aspetti psicologici, emozionali, sociali e spirituali per costruire intorno al malato e alla sua famiglia una "rete" di protezione e di cura. I trattamenti di supporto psicologico per la famiglia del paziente oncologico sono centrate su skills comunicativi, sulla facilitazione dell''espressivit� emozionale, sulla ristrutturazione cognitiva dell''esperienza di malattia e sulla condivisione empatica dei sentimenti di frustrazione, rabbia e dolore scatenati dalla malattia. Un''ulteriore funzione della Psiconcologia � la ricerca, volta alla chiarificazione delle possibili reazioni tra psiche e soma nei confronti della patogenesi dei tumori e della loro evoluzione. Nella parte centrale della tesi sono state esaminate le variabili psicologiche e sociali, capaci di condizionare a vari livelli l''esposizione degli individui a fattori di rischio per le neoplasie.L''assunzione fondamentale del modello bio-psico-sociale � che la condizione di salute o di malattia sia la conseguenza dell''interazione tra fattori biologici, psicologici e sociali. Il meccanismo biologico principalmente chiamato in causa nel correlare tra loro ambiente, reazione da stress, aspetti psichici e cancro � rappresentato dal sistema immunitario.Ne deriva una lettura della patogenesi del cancro che corrisponde a un modello di rischio di tipo multifattoriale. In tal senso, l''influenza negativa di alcuni fattori psichici, in particolare della risposta patologica allo stressor, sembrerebbe dimostrarsi un co-fattore di rischio per l''insorgenza del cancro.Diversi autori, tra i quali LeShan e Kissen, occupandosi della possibile struttura di personalit� del malato di cancro, hanno correlato la malattia a tipiche situazioni psicologiche individuali, quali le esperienze di perdita o l''incapacit� di espressione, sul piano emozionale, delle pulsioni aggressive, ipotizzando peculiari tratti di personalit� del paziente oncologico.La rabbia nasce come risultato della restrizione alla nostra spontaneit�, restrizione che facciamo per sopravvivere alle minacce che percepiamo provenire dal mondo esterno. La repressione della rabbia � un processo che indebolisce la vitalit� e, se non viene superata, la chimica del nostro corpo potrebbe subire alterazioni con l''induzione a modificazioni cellulari e neoplasie.Nel caso di uno stato neoplastico gi� conclamato, il processo catartico di espulsione delle tensioni, della rabbia e della paura potrebbe favorire processi di rallentamento della diffusione neoplastica, coadiuvare al meglio le terapie oncologiche e aumentare la capacit� di sopportazione di cure pesanti come la chemioterapia.
