Introduction: -- The impact of regular physical training on the cardiovascular system is well known in Caucasian athletes but there is paucity of data in athletes of other ethnic origins, especially athletes of African/ Afro-Caribbean origin (black) who form an important component of the athletic population in Western world. -- Methodology: -- Between 2005 and 2008, 300 highly trained black male athletes (mean age 20.5 ± 5.8 years) underwent 12-lead ECG and 2-dimensional (2D) transthoracic echocardiography. The results were compared with control groups comprising of 150 black male sedentary individuals, 150 white male sedentary individuals and 300 highly trained white male athletes of similar age and size that participated in identical sporting disciplines. Athletes exhibiting left ventricular wall thickness > 12 mm were subjected to exercise stress testing and 48-hour ambulatory ECG recordings to look for broader phenotypic features of HCM. -- Results: -- Black athletes had a greater magnitude of left ventricular wall thickness than their Caucasian counterparts (11.3 mm ± 1.6mm Vs 10mm ± 1.5 mm; p0.001). In absolute terms, 54 (18%) black male athletes had a LV wall thickness 12 mm compared with 12 (4%) white male athletes and 3% of black male athletes exhibited LV wall thickness s> 15 mm as compared to none of the white male athletes. All athletes with left ventricular hypertrophy (LV wall thickness > 12 mm) had associated dilated LV cavity, normal diastolic function, exercise stress test and ambulatory EGG recording to suggest physiologic LVH. In addition, black athletes also had higher prevalence of T-wave inversion that overlap with those observed in hypertrophic cardiomyopathy and arrhythmogenic right ventricular cardiomyopathy than white athletes. Sudden death in athletes is a global problem. Although it is a relatively rare phenomenon (1/100,000 persons), when it does occur, it is often as an incomprehensible event. In fact, it strikes subjects who presumably should be much healthier than the general population. In the previous 20 years, many authors have studied this problem in an attempt to understand the causes and prevent these events, and it has been determined that, in the vast majority of cases, athletes who die suddenly have an underlying heart disease (arrhythmogenic cardiomyopathy, hypertrophic cardiomyopathy, coronary anomalies, channelopathies, etc.). In most cases these diseases do not produce major symptoms and do not preclude sports activity even at the highest levels, although they do increase the incidence of sudden death. How to discover these diseases in asymptomatic athletes is a hotly debated issue. In particular, there is controversy as to whether all athletes should undergo detailed medical screening, including electrocardiogram, or whether the costs of this screening are too high in relation to the event incidence. The purpose of this book is to accurately analyze the causes of sudden death in athletes and to provide cardiologists and sports physicians with useful tips on how to identify at-risk individuals.
