The Baby-friendly Hospital Initiative (BFHI) is a global effort launched by WHO and UNICEF to implement practices that protect, promote and support breastfeeding. It was launched in 1991 in response to the Innocenti Declaration. The global BFHI materials have been revised, updated and expanded for integrated care. The materials reflect new research and experience, reinforce the International Code of Marketing of Breast-milk Substitutes, support mothers who are not breastfeeding, provide modules on HIV and infant feeding and mother-friendly care, and give more guidance for monitoring and reassessment.

Women have shared breast milk for eons, but in White Gold, Susan Falls shows how the meanings of capitalism, technology, motherhood, and risk can be understood against the backdrop of an emerging practice in which donors and recipients of breast milk are connected through social media in the southern United States. Drawing on her own experience as a participant, Falls describes the sharing community. She also presents narratives from donors, doulas, medical professionals, and recipients to provide a holistic ethnographic account. Situating her subject within cross-cultural comparisons of historically shifting attitudes about breast milk, Falls shows how sharing “white gold”—seen as a scarce, valuable, even mysterious substance—is a mode of enacting parenthood, gender, and political values. Though breast milk is increasingly being commodified, Falls argues that sharing is a powerful and empowering practice. The statement begins by listing 10 important steps to successful breast-feeding intended for application in every facility providing maternity services and care for newborn infants. Readers are told that mothers should be helped to breast-feed within a half hour of birth that newborn infants should be given no food or drink other than breast milk unless medically indicated and that rooming-in should be practised 24 hours a day. Particularly practical is a section devoted to individual care, which spells out procedures to follow at five important stages from prenatal history-taking and counseling through care during and after delivery to what to do when a mother is discharged from the health care facility. Readers are informed that the risk of neonatal infection is in fact greater in the closed environment of a nursery than when infants remain with their mothers and that exclusive on-demand breast-feeding should be the norm throughout the clinic or hospital stay. The booklet concludes with a 20-point check-list that maternity wards and clinics can use to gauge how well they are protecting promoting and supporting breast-feeding.

Interest in implementation research is growing, largely in recognition of the contribution it can make to maximizing the beneficial impact of health interventions. As a relatively new and, until recently, rather neglected field within the health sector, implementation research is something of an unknown quantity for many. There is therefore a need for greater clarity about what exactly implementation research is, and what it can offer. This Guide is designed to provide that clarity. Intended to support those conducting implementation research, those with responsibility for implementing programs, and those who have an interest in both, the Guide provides an introduction to basic implementation research concepts and language, briefly outlines what it involves, and describes the many opportunities that it presents. The main aim of the Guide is to boost implementation research capacity as well as demand for implementation research that is aligned with need, and that is of particular relevance to health systems in low- and middle-income countries (LMICs). Research on implementation requires the engagement of diverse stakeholders and multiple disciplines in order to address the complex implementation challenges they face. For this reason, the Guide is intended for a variety of actors who contribute to and/or are impacted by implementation research. The second half of the Guide (5-7) detail the various methods and study designs that can be used to carry out implementation research, and, using examples, illustrates the application of quantitative, qualitative, and mixed-method designs to answer complex questions related to implementation and scale-up. Maternity nursing practice is changing across Canada with the movement toward becoming "baby friendly." The World Health Organization (WHO) recommends the Baby-Friendly Hospital Initiative (BFHI) as a standard of care in hospitals worldwide. Very little research has been conducted with nurses to explore the impact of the initiative on nursing practice. The purpose of this study, therefore, was to examine the process of implementing the BFHI for nurses. The study was carried out using Corbin and Strauss's method of grounded theory. Theoretical sampling was employed, which resulted in recruiting and interviewing 13 registered nurses whose area of employment included neonatal intensive care, postpartum, and labour and delivery. The data analysis revealed a central category of resisting the BFHI. All of the nurses disagreed with some of the 10 steps to becoming a baby-friendly hospital as outlined by the WHO. Participants questioned the science and safety of aspects of the BFHI. Also, participants indicated that the implementation of this program did not substantially change their nursing practice. They empathized with new mothers and anticipated being collectively reprimanded by management should they not follow the initiative. Five conditions influenced their responses to the initiative, which were (a) an awareness of a pro-breastfeeding culture, (b) imposition of the BFHI, (c) knowledge of the health benefits of breastfeeding, (d) experiential knowledge of infant feeding, and (e) the belief in the autonomy of mothers to decide about infant feeding. The identified outcomes were moral distress and division between nurses. The study findings could guide decision making concerning the implementation of the BFHI.
