Primary health care is the bedrock of any countrys health care system. It is aimed to provide the individuals and the communities with protective, preventive, restorative and rehabilitative health services within the available resources. However there is a growing lack of confidence and trust by the public in all health services at this level of care, resulting from the poor state of the facilities and low standards of delivery. This book therefore sought to explore and highlight those barriers affecting the utilization of primary health care services in a low income nation like Nigeria. It also aimed to serve as a basis for understanding and appreciating the complex factors affecting the utilization of these essential services, and how these factors translate into a deterioration of a nation's health indices. This report analyzes health care utilizations as they relate to impairment severity and SSA's definition of disability. Health Care Utilization as a Proxy in Disability Determination identifies types of utilizations that might be good proxies for "listing-level" severity; that is, what represents an impairment, or combination of impairments, that are severe enough to prevent a person from doing any gainful activity, regardless of age, education, or work experience.

This paper, based on quantitative surveys at the level of primary health care facilities, health care personnel, and households in their vicinity, aims at understanding the performance of primary health care providers in four states in Nigeria. As possible ways to improve performance, the paper concludes that clearly defining lines of responsibility, implementing performance-based financing of local governments and providers, and collecting, analyzing, and sharing information are some options that can help realign incentives and improve accountability in the service delivery chain and service provision. This working paper was produced as part of the World Bank s Africa Region Health Systems for Outcomes (HSO) Program. This book introduces and explicates the end-to-end resource tracking and management (RTM) framework, which includes five components that determine effective and efficient financing for PHC: resource mobilization, allocation, utilization, productivity, and targeting.In addition, this book compiles detailed results from the most recent RTM-based resource tracking efforts for PHC in selected countries. This is to demonstrate how the RTM framework can be used to bring a set of separate resource tracking efforts at different stages of flow of funds into a comprehensive process with an end-to-end 'storyline'. In the United States (US), where access to and affordability of healthcare remains a critical challenge, emergency departments (ED) have increasingly served as a source for primary care and a portal to hospital admissions. Although barriers to timely primary care and lack of insurance are considered influential factors, the inter-relationship between these issues and ED use is not well-understood. We, therefore, examined the relationship between self-reported insurance status, perceived barriers and ED use among non-institutionalized US adults. We analyzed data from 378,502 participants of the 1999 to 2011 National Health Interview Survey. The following were considered as barriers to timely primary care as experienced during the past year: unable to get through on telephone, unable to obtain appointment soon enough, long wait in the physician's office, limited clinic hours, and lack of transportation. Individuals were classified by source of insurance into Private, Medicare, Medicaid, Other insurance, and No insurance. National estimates were obtained using appropriate sampling weights. Logistic regression models were used to estimate odds ratios (OR) and 95% confidence intervals (CI) for [greater then/equal to] 1 ED use and to test for the interaction between self-reported barriers and source of insurance. Overall, 10.0% of adults had [greater then/equal to] 1 barrier to care and 20.2% reported [greater then/equal to] 1 ED visit in the past year. Insurance profile was as follows: Private (57.8%), Medicare (17.7%), Uninsured (16.3%), Medicaid (5.0%), and Other Insurance (2.6%). Compared to those without any barrier, adults who experienced [greater then/equal to] 1 barriers were more likely to report [greater then/equal to] 1 ED visits in the past year (32.6% vs. 18.8%). In multivariate models that included both barriers and source of insurance, the odds of an ED visit was greater among adults with [greater then/equal to] 1 vs. no barriers (OR [95%CI] - 2.01 [1.95-2.07]), and varied across source of insurance (OR [95%CI]-Private: reference; Medicare: 1.76 [1.72 - 1.81]; Medicaid: 3.11 [3.00-3.23]; Other: 1.83 [1.73 - 1.94]; Uninsured: 1.26 [1.23 - 1.30]). The magnitude of association between barriers and ED use varied across insurance categories. Among non-institutionalized adults in the US, the source of insurance was seen to influence the association between perceived barriers to timely care and ED use. Further analyses are planned to determine whether this relationship persists after controlling for socio-demographic and clinical characteristics.

Behavioral health conditions, which include mental health and substance use disorders, affect approximately 20 percent of Americans. Of those with a substance use disorder, approximately 60 percent also have a mental health disorder. As many as 80 percent of patients with behavioral health conditions seek treatment in emergency rooms and primary care clinics, and between 60 and 70 percent of them are discharged without receiving behavioral health care services. More than two-thirds of primary care providers report that they are unable to connect patients with behavioral health providers because of a shortage of mental health providers and health insurance barriers. Part of the explanation for the lack of access to care lies in a historical legacy of discrimination and stigma that makes people reluctant to seek help and also led to segregated and inhumane services for those facing mental health and substance use disorders. In an effort to understanding the challenges and opportunities of providing essential components of care for people with mental health and substance use disorders in primary care settings, the National Academies of Sciences, Engineering, and Medicine's Forum on Mental Health and Substance Use Disorders convened three webinars held on June 3, July 29, and August 26, 2020. The webinars addressed efforts to define essential components of care for people with mental health and substance use disorders in the primary care setting for depression, alcohol use disorders, and opioid use disorders; opportunities to build the health care workforce and delivery models that incorporate those essential components of care; and financial incentives and payment structures to support the implementation of those care models, including value-based payment strategies and practice-level incentives. This publication summarizes the presentations and discussion of the webinars.
