This book incorporates the most up-to date information related to mechanisms and treatment of cardiac arrhythmia. Many of the topics discussed in this text reflect very recently undertaken research directions including genetics of arrhythmias, cell signalling molecules as potential therapeutic targets and trafficking to the membrane. These new approaches and implementations of anti-arrhythmic therapy derive from many decades of research as outlined in the first chapter by distinguished Professors Michael Rosen (Columbia University) and Michiel Janse (University of Amsterdam). The text covers changes in approaches to therapy over time, arrhythmias in multiple cardiac regions and over many scales, from gene to protein to cell to tissue to organ.

A significantly expanded third edition, this book provides a comprehensive and concise overview of cardiac arrhythmias and their ECG/telemetry manifestations, including the principles of cardiac electrophysiology, current concepts of pharmacology, clinical features, diagnoses, and state-of-the-art treatments. Additionally, the book emphasizes decision-making strategies in approaching each individual patient and the application of technical innovations in specific clinical situations. Organized into eight parts, beginning chapters introduce the concepts and principles of cardiac electrophysiology, unique rhythms, and ECG waves/signs. These chapters are designed to integrate emerging knowledge in basic science and clinical medicine. Subsequent chapters focus on the diagnosis of a variety of cardiac arrhythmias using non-invasive methodology. Throughout the book, chapters continue to analyze pharmacological and other approaches to therapy of specific arrhythmias, including supraventricular tachycardias, atrial fibrillation and flutter, ventricular arrhythmias, and bradyarrhythmias. Finally, the book closes with coverage on inherited cardiac arrhythmia syndromes including the long, short QT, and J-wave syndromes, catecholaminergic polymorphic ventricular tachycardia, and arrhythmogenic right ventricular cardiomyopathy. The third edition of Management of Cardiac Arrhythmias, is an essential resource for physicians, residents, fellows, and medical students in cardiology, cardiac surgery, vascular surgery, cardiac electrophysiology, and cardiac radiology.

This completely updated and expanded 2nd Edition provides all the information needed in order to understand the complex molecular, cellular and genetic mechanisms that underlie normal and abnormal cardiac rhythms. Its goal is to help physiologists and clinicians alike develop better preventive and treatment strategies. The respective chapters cover a broad range of topics, including the role of specific ion channels and transporters, gap junctions, intracellular Ca2+ handling in pacemaker activity, impulse conduction, and the activity of atrial and ventricular myocardium. Special emphasis is placed on the unique electrophysiology of specialized pacemaking cells and conducting fibers. In turn, several types of inherited and acquired ventricular arrhythmias are presented in detail. Clinicians will especially appreciate the updated information on even common disorders like atrial fibrillation and their impacts on human physiology. In closing, the book goes “one step further” and considers future prospects, e.g. novel antiarrhythmic agents and new concepts like optogenetics, in the context of heart rate regulation. Accordingly, it offers readers a solid reference guide to everything they need to know about human heart rate and rhythm.

This single-source reference/text is an authoritative, up-to-date, and multidisciplinary presentation of basic, applied, and clinical approaches to the diagnosis, treatment, and management of cardiac arrhythmia and the prevention of sudden cardiac death-providing essential concepts for new approaches to pharmacologic and electrical therapies. Over 50 leading physicians, scientists, and engineers integrate their research knowledge into a solid foundation of fundamentals in innovative new ways to promote an understanding of cardiac arrhythmias on a multilevel basis that spans the full range of topics from genes to therapy prevention. From the regular rhythm of the heart to the irregular, chaotic states that characterize fibrillation and tachyarrhythmias, Foundations of Cardiac Arrhythmias explores the ionic and molecular basis of electrogenesis and its control within different types of cardiac cells clarifies the molecular and biochemical regulation of cell-to-cell conduction that will help facilitate development of the next generation of antiarrhythmic drugs considers genetic determinants that influence the onset of sudden death in rare and acquired heart disease explores recent insights into macroscopic, three-dimensional interactions implicated in the genesis of malignant ventricular tachycardias surveys population studies that reveal new information about the relevance of higher frequency polymorphisms and variations in molecules involved in cardiac control discusses the role of cardiac ablation and the use of pacemakers and defibrillators, including new concepts in device design discusses promising new advances with noninvasive markers of arrhythmia risk that are helping to identify patients at risk for sudden death Containing nearly 2300 key literature citations and over 300 helpful drawings, photographs, equations, and tables, Foundations of Cardiac Arrhythmias serves as a thorough and inspiring reference for clinical and research cardiologists, clinical and basic electrophysiologists, pharmacologists, molecular and cell physiologists, biologists, biochemists, molecular geneticists, biomedical and electrical engineers, and biophysicists, as well as an important text for graduate students, residents, and fellows in these disciplines.

Pathological heart rhythms are a major health issue. In this book experts from various fields provide an important context for understanding the complicated molecular and cellular mechanisms that underlie normal and pathophysiological cardiac rhythms. Individual chapters cover a full range of topics, including the ionic basis of pacemaking, the role of specific channels and transporters in sinoatrial node pacemaking, altered intracellular Ca2+ handling in response to disease, computer modeling of the action potentials of pacemaker and working cardiomyocytes, genetic and molecular basis of inherited arrhythmias and a review of established and novel antiarrhythmic agents. Due to the key importance of the specialized pacemaker cells and tissue (sinoatrial and atrioventricular nodes) in maintaining heart rate and rhythm, special emphasis is placed on the peculiar electrophysiology of these cells.

Asked to organize an international meeting on Propafenone (Ryt monorm) we had to question ourselves, whether to accept this offer just as interested physicians or in our capacity as chairmen of the study group on arrhythmias and intracardiac electrography of the European Society of Cardiology. Considerung the "pros and contras" and well aware that one can easily be regarded as "product promoter" we, nevertheless, choose to organize the Symposion in association with the study group. We felt justified to do so for a number of reasons: Arrhyth mias and their treatment as well as the tools to detect the first and to control the latter are well within the scope of the study group. An international symposion provided the opportunity to have the "state of the art" in arrhythmias and antiarrhythmic treatment thoroughly reviewed by internationally renowned investigators, thus serving an educational purpose. Furthermore it was the intention and aim of the meeting to have what is known about a fairly new drug presented in a critical manner and to point out what is not known, but should be known. It is in that context the presentations of the free session should be judged. To the careful reader it will become apparent that much which was not said, can be read "between the lines".

The book describes novel and state-of-the-art methods for differentiating between the major types of arrhythmia, structural abnormalities and current practice guidelines and determination of risk stratification associated with sudden cardiac death. A particular focus is on arrhythmias associated with atrial fibrillation and includes details of associations with cardiac disease, current detection, analysis and imaging and future perspectives.
