Jumlah pasien yang menjalani tindakan bedah jantung dewasa ini terdapat kecenderungan meningkat. Hal ini didorong oleh makin baiknya fasilitas diagnostik, bertambahnya jumlah tenaga medis ahli, makin baiknya infrastruktur rumah sakit, dan obat-obatan yang lengkap serta kerja sama antarbagian di dalam rumah sakit maupun antarrumah sakit sehingga pasien pasien dengan kelainan jantung yang membutuhkan tindakan koreksi dengan bedah dimungkinkan untuk mendapatkan pelayanan secara paripurna. Buku ini menjadi salah satu panduan untuk pasien-pasien yang menjalani tindakan bedah jantung terbuka, apakah dikarenakan penyakit jantung koroner, penyakit katup jantung, penyakit jantung bawaan, dan kelainan jantung lainnya. Keputusan untuk melakukan tindakan bedah diambil setelah dilakukan pengkajian yang melibatkan lintas ahli, spesialis jantung dan pembuluh darah, spesialis bedah thorax dan kardiovaskular, spesialis anestesi, spesialis jantung dan pembuluh darah subspesialis prevensi, dan rehabilitasi kardiovaskular. Tahap-tahap untuk menjalani bedah jantung diawali dengan persiapan, tindakan operasi, dan pemulihan pascaoperasi. Pada tahap persiapan kadang diperlukan konsultasi dengan spesialis THT dan dokter gigi, serta spesialis paru. Juga dilakukan latihan batuk dan pernapasan efektif. Pada pascaoperasi dilakukan rehabilitasi fase 1, fase 2, dan fase 3 di bawah bimbingan dokter. Masing-masing fase memiliki komponen yang komprehensif dengan tujuan untuk mengembalikan pasien pada kapasitas yang memungkinkannya untuk bisa beraktivitas seperti orang sehat. Pasien juga diberikan edukasi tentang perawatan luka, gizi dan obat-obatan. Juga diberi edukasi tentang aktivitas apa saja yang dianjurkan dan dihindari selama pemulihan, kapan boleh mulai bekerja, mulai mengendarai kendaraan, dan tips saat bepergian. Semoga buku ini bisa memberi pemahaman yang lebih baik bagi pembaca yang memerlukan.

The motivation for this book was the recognition by two of us (RL and TL) that, despite our training as cardiologists, we had a limited understand ing of many aspects of the surgical management of our patients. Conversa tions with other cardiologists at our own institution and at other medical centers around the country convinced us that many of our colleagues shared our uncertainty about the details of surgical procedures and the factors that determine decisions in the perioperative period. As surgical techniques continue to evolve, this knowledge gap can become only more severe. We therefore compiled information about cardiac surgery that might be useful for cardiologists and other nonsurgeons. We asked cardiologists what questions they would like to ask their surgical colleagues and provided these questions to the authors of the chapters. Thus, the goal of these chapters is to help nonsurgeons answer their own questions about common cardiac procedures, and to help prepare these physicians to address the questions raised by their patients. Nearly a century has passed since Rehn performed the first successful cardiac operation by closing a right ventricular stab wound in a gravely ill patient. Moreover, it has been more than fifty years since Gross successfully corrected the first congenital cardiac malformation in 1938 by suture ligation of a patent ductus arteriosus. The introduction of the Blalock operation for tetralogy of Fallot by Blalock in 1944 greatly advanced the management and prognosis of a critically ill group of cardiac patients, and the success of this procedure further stimulated the development of concepts and techniques for the surgical management of other severe congenital cardiac defects. Until the successful use of extracorporeal circulation by Gibbon in 1953, it was often necessary to perform cardiac operations which were palliative rather than curative procedures. With the advent of additional new and improved techniques, correction of many hitherto incurable cardiac disorders became possible and reoperation under these circumstances became frequent. Cardiac surgery is very fortunate in having two master surgeons, whose distinctive contributions and clinical proficiency are recognized worldwide, to edit this extraordinary and unique text. They have placed emphasis on a number of specific complications of primary cardiac procedures which lead to the necessity for reoperation. Problems associated with postoperative infections, thrombotic dis orders, stenoses of suture lines, deterioration of prosthetic materials and mechanical valves, rejection of transplanted organs and tissues, and a host of additional complications are described together with their appropriate surgical management.
