Objectives: We propose a procedural approach (PPS: predictive provisional stenting) to predict the success of provisional stenting (PS) in coronary bifurcation lesions. Background: The strategy of stenting the main vessel (MV) with side branch (SB) PS is the current favored approach for percutaneous coronary interventions of bifurcation lesions. However, angiographic results of SB ballooning in PS are quite variable and may require switch of 1-stent into 2-stents strategy which can be technically more difficult to perform once the MV stent has been implanted. Methods: PPS approach is based on the pre-stenting angiographic analysis of the SB ostium response both the MV pre-dilatation using a balloon matched to the size of the MV stent and to additional ballooning of the SB if its residual stenosis is ≥50%. This algorithm was used in 50 bifurcations lesions. Results: Final angiographic success was 100% for both the MV and the SB. Based on the PPS algorithm, PS was converted before MV stenting into 2-stents strategy in 7 cases (14%) due to SB ostium significant residual stenosis after MV balloon not corrected by SB ballooning. PS was successful in ail the remaining 43 lesions (86%) in which it was predicted to be successful by PPS. At 12-months follow-up, no major cardiovascular adverse event including death, myocardial infarction, stroke, clinically-indicated lesion vessel revascularization occurred in any patient (0%). Conclusion: Our approach allows to determine which bifurcation lesions can be successfully treated by PS and confirms that PS can be successful in a majority of bifurcation lesions.

Hilar cholangiocarcinoma is one of the most common disorders of biliary obstruction. Various stent configurations have been used for the palliative treatments of the biliary bile duct, including dedicated Y-shaped (DYS) stent and two-piece stent forming a Y-shape, also referred to as a separate Y-shape (SYS) stent. The mechanical characterizations of the bifurcating stents are scare. The aim of this study is to characterize the mechanical performance of both DYS and SYS self-expanding Nitinol stents using finite element method for better understanding of the biomechanical environment changes on the bile duct and its clinical implications. The crimping and expansion of both stents in the bile duct was simulated. The hyperelastic constitutive model of porcine bile ducts was constructed from tensile tests. The acute performance of both stents was quantified and compared in terms of the lumen gain, wall deformation and stress. Results showed the SYS stent provided better lumen gain, but larger stress and strain on the bile duct wall, compared to the DYS. A larger wall stress is generally associated with tissue remodeling and thus chronic complications. Also, the efficacy of the two Y-stenting methods was compared with respect to plastic strain distribution, stent migration, and delivery challenges. This work might facilitate the pre-surgery planning and new stent design.

The present study used a fractal bifurcation bench model to compare 6 optimization sequences for coronary bifurcation provisional stenting, including one novel sequence without kissing balloon inflation (KBI), comprising initial proximal optimizing technique (POT) + sidebranch inflation (SBI) + final POT, and called “re-POT”. In provisional bifurcation stenting, KBI fails to improve the rate of major adverse cardiac events. Proximal geometric deformation increases the rate of in-stent restenosis and target lesion revascularization. A bifurcation bench model was used to compare KBI alone, KBI after POT, KBI with asymmetric inflation pressure after POT, and 2 sequences without KBI: initial POT plus SBI, and initial POT plus SBI with final POT (called “re-POT”).
