This volume contains the proceedings of the Second International Symposium on Biology of Brain Tumour. The first Symposium was held in 1979 at Gardonne Riviera, Italy. This meeting was planned in order to coincide with the lOOth Anniversary of the first reported operation for glioma in London on November 25, 1884. Since the first meeting, the field of neuro-oncology has made remarkable progress in understanding both basic and clinical factors of significance to patients with brain tumor. While the earlier meeting dealt to a large extent with clinically oriented studies, this symposium was more heavily weighted toward the biology of brain tumour and improving our understanding at the physiologic, biochemical, pharmacologic, and cellular level. The meeting was divided according to scientific content into presentations and discussions as well as posters for more leisurely viewing, so as to allow the main themes of the meeting to sequentially develop. The first session dealt extensively with neuro-oncology at the molecular level and included considerable discus sion of material related to the babic biochemical milieu in which tumors originate, proliferate, and eventually destroy the brain. Classic neuropathology has been the mainstay of tumor identification and characteriza tion, however, the process of classification has become much more complex. The availability of a variety of new tools has allowed investigation into the validity of the more traditional classification systems as well as the development of newer biologically related concepts.

The third American edition has been completely revised and expanded, although parts of the text of the second edition have been included. I wish to acknowledge once again the excellent translation of the former two editions by Dr. ALAN B. ROTHBALLER and the late Dr. JERZY OLSZEWSKI. With this edition I have followed the general theme of the original German edition published in 1951. However, I have tried to consider modern techniques and the many new publications on the subject of brain tumors. Meanwhile, an early desire of mine has been fulfilled by the completion and publication of a classification which can be understood worldwide and hopefully be used widely, namely, the classi fication of the World Health Organization: Histological Typing of Tu mours of the Central Nervous System (1979). The classification which I used in the 1951 edition is very close to the final pattern of that accepted by the World Health Organization (WHO), since both follow the line of the BAILEY and CUSHING classifica tion of 1926/1930. To consolidate our old concepts and experiences we have reclassi fied our collection of 9000 cases with the assistance of my co-workers Dr. M. FUKUI, Dr. A. SATO. Dr. E. SCHARRER, Dr. E. SIMON, and Dr. J. SZYMAS. In the last decade two large atlases have been published, one called an Atlas of the Histology of Brain Tumors 1 (in six languages) and a second one called an Atlas of the Gross Neurosurgical Pathology 2.

Exciting new developments and discoveries of the last two decades are beginning to shed light on the complex biology of brain tumors and are advancing our understa- ing of the cellular and molecular processes involved in their initiation, progression, and clinical and biological behavior. The disease process in brain tumors is quite complex and the resulting tumors are characterized by a high degree of biological and clinical diversity. Thus, despite the advances of the last two decades, prognosis for patients with malignant brain tumors remains abysmal. Significant progress in the diagnosis, treatment and, ultimately, prevention of these tumors will require both the timely h- nessing of the advances in basic and clinical brain tumor research, and a continuing concerted effort at increasing our understanding of brain tumor biology, in particular, the molecular genetic changes and perturbations of cellular pathways involved in brain oncogenesis and which drive the biological and clinical behavior of the tumors. Brain tumor diagnosis and prognosis, which is still largely based on histopathology and other clinical criteria, will, in the future, acquire a significant molecular component, with the incorporation of knowledge of genes that are mutated, over-expressed, deleted, silenced, or functionally altered in the tumors. Treatment strategies for brain tumors, rather than being empirical, will be rationally developed based on an understanding of the cellular and molecular mechanisms and targets that have been activated, suppressed, or otherwise altered.
