There are many different types of cancer. Some are very common; others are rare. Having some similar features, each cancer has its own characteristics and symptoms, which vary according to the type of cancer and where it is in the body. The treatment selected may be influenced by the patient's age, concurrent health problems, life expectancy, characteristics of the individual's cancer, the patient's goals of treatment, and the bias of the treating physician. In preparing this book, we have considered and reflected on the concept of the recent advances in cancer management and therapy as well as molecular applications in cancer therapy. The readers can find detailed information about the treatment and management of specific cancers in these book chapters.

The optimal management of breast cancer patients relies on the expertise of a team of medical specialists including radiologists, surgeons, radiation therapists and medical oncologists. Much of the progress in breast cancer management made over the last several years reflects the translation of observations made in the laboratory to the clinic. Critically evaluating the impact of new treatment approaches relies on a commitment to well-designed clinical trials. In this volume, Advances in Breast Cancer Management, a renowned group of breast cancer experts have been asked to provide their perspective on management issues that directly effect patients on a day-to-day basis. Dr. Melody Cobleigh discusses the consequences of estrogen deprivation and the ways of ameliorating secondary symptoms and the potential long-term morbidity. Drs. Haigh and Guiliano review the sentinel lymph node biopsy technique including results from their extensive experience. Dr. Abram Recht places into perspective the potential benefit of post-mastectomy radiotherapy and reviews recent trials that address this issue. Dr. Dennis Slamon takes from us from the laboratory to the clinic in explaining the development of Herceptin as a paradigm for therapy targeted to specific molecular characteristics of breast cancer tumor cells. Drs. Nieto, Shpall, Crump and Pritchard offer different perspectives on the future of high-dose chemotherapy with stem cell transplantation as a treatment for breast cancer patients. Drs.

The editor of Skin Cancer Management: A Practical Approach, Dr. Deborah MacFarlane, gathers experts in selected techniques related to the assessment and management of skin cancer and has them critically review the existing literature in light of their considerable experience delivering care. The authors make recommendations for the best way to perform procedures. The tables provided in each chapter then become a manual of how to perform these procedures, and may in time be adopted by the wider universe of dermatologists as the standard of performance. The detailed descriptions of technique and treatment pearls lead the novice through the sequence of events in a way that instills confidence in their ability to safely perform the procedure. An example of the painstaking expla- tions is found in Chapter 5, Intralesional and Perilesional Treatment of Skin Cancers. The reader is advised to place eye protection on the patient and those performing the injection of methotrexate into a keratoacanthoma with a central crust. Rest assured that there will be a spray or stream of methotrexate emitted from the crusted area. Having eye protection will prevent methotrexate from accidentally getting into someone’s eye. Since we all learn to assimilate new information by taking action on the recommendations that we read, it would be a good idea for the physician to create, where relevant, a checklist for each procedure in the text. The checklist can be given to the office staff to set up the equipment for the procedure.

Cancer is a general term which applies to a broad spectrum of malignant diseases. For most patients, the diagnosis of cancer evokes images of intractable pain, mutilation, and impending death. Many of these feelings are derived from distortions of one's experiences with relatives or friends, or are based on misconceptions or myths about the disease itself. Fortunately, technologic and therapeutic advances have created a more optimistic future for the cancer patient. Health care teams, specializing in oncology, are defining and attending to the special needs of cancer patients. They are developing cancer management profiles for each patient which involve not only the primary diagnostic and therapeutic aspects of the disease but also its secondary effects on physiologic and psychologic processes. The purpose of this primer is to function as a general guide for managing all stages of cancer-from the early curable tumor to advanced metastasis. Therapy for such patients may require surgery, chemotherapy, radiation therapy, or a combination of modalities including immune therapy. This text will focus on cancer care and the role of the staff in the following general areas: (1) the communication of the diagnosis and the characteristics of the disease; (2) the application of the therapeutic modalities; (3) the management of secondary symptoms such as pain; and (4) the development of support services such as psychological counseling.

This book presents expert opinions on a variety of key topics related to the management of breast cancer, with a focus on the implications of recent advances and research findings for clinical practice. It also explores the controversy regarding mammography screening and reviews the contribution of new imaging modalities. Considerable attention is paid to developments in surgical procedures, including the potential for the safe and effective use of sentinel lymph node dissection alone—even in patients with positive nodes—and to the advantages and contraindications of new radiotherapy techniques. Genetic aspects are discussed in detail, including an assessment of the role of genetic testing and the potential impact of genetic signatures on breast cancer management. New systemic strategies, such as anti-HER2 therapy, endocrine agents, and agents to reverse endocrine resistance, are considered, and the optimal use of chemotherapy for early-stage and advanced-stage disease is addressed. In closing, the book shares important new insights into lifestyle risk factors, risk reduction strategies, and survivor issues, including sexual dysfunction and fertility maintenance.
