The significant advances in medical, surgical and social care during the last two decades in the western world have resulted in a considerable prolongation of life expectancy. However, on the other hand, aged populations present a high incidence of neoplasias ideally requiring surgical or multidisciplinary therapeutic strategies. The decision making for providing the appropriate surgical care in these patients is a difficult and multivariant task. This book examines cancer surgery in elderly patients, as well as a need for a more 'personalised' approach and a thorough discussion of their disease treatment and status of life.

As the population ages, clinicians are facing an increasing number of elderly patients with colorectal cancer. These patients pose unique challenges as they have more comorbidities and lower functional reserves. In addition, the treatment goals may differ from those in younger patients. This book discusses in depth the different aspects of management of colorectal cancer in the elderly. After the provision of pertinent background information on the normal physiology of aging, screening and diagnosis are discussed. Subsequent chapters focus on a range of issues associated with the surgical and perioperative care of these patients and with adjuvant treatment and palliative care. Each chapter provides helpful take-home messages in bullet point form, and numerous informative figures and tables are also included. The authors are surgeons, physicians, anesthetists, geriatricians, oncologists, and allied health professionals with extensive experience in the field.

This volume offers a comprehensive and up-to-date overview of the major surgical procedures performed on elderly patients. With advancing age the conditions that require surgery normally increase in incidence, and today all surgeons around the globe can be considered “geriatric surgeons”. The latest anesthesiological procedures, the technical improvements and an increased medical knowledge of the comorbities impact have brought extraordinary advances in this field, but this should not allow us to forget that a careful assessment of elderly patients is necessary before undertaking surgery, since also the simplest surgical procedures could alter the delicate psychophysical equilibrium of these “frail” patients. Introduced by a demographic panorama and analysis, the book describes the anesthesiological management, the preoperative assessment and preparation, the perioperative care, and hemostasis control connected with the main surgical pathologies: gastrointestinal, endocrine, neoplastic, vascular and hepato-biliary, as well with traumatic and non-traumatic emergencies. It will be an invaluable, easy-to-use resource for clinicians, researchers and trainees alike.

Outcomes Of Care In Elderly Patients With Lung Cancer Treated With Surgery written by Cardinale B. Smith and has been published by this book supported file pdf, txt, epub, kindle and other format this book has been release on 2015 with categories.

Lung cancer is predominantly a disease of older adults and the leading cause of cancer-related deaths in the elderly. Current recommendations for surgery are to perform an anatomic lobectomy however, patients who are not candidates for full resection due to impaired pulmonary function or comorbidities are usually treated with a limited resection. However, there is very limited data comparing outcomes of stage I patients treated with these procedures making choices regarding the best surgical approach for patients undergoing limited resection more difficult. Limited resections can be performed via video-assisted thorascopic surgery (VATS) and have been performed with increasing frequency to treat early-stage non-small cell lung cancer (NSCLC). However, data supporting these recommendations are inadequate and very few studies focused on the elderly, one of the groups who could benefit the most from VATS, a less aggressive surgical approach. Compelling evidence from several studies suggest that cancer patients whose surgical resections are performed in hospitals with large case volumes have better outcomes than patients treated in hospitals with low case volumes. Similarly, some studies suggest that the case volume of individual surgeons is an equally important determinant of patients’ outcomes. Given that VATS is a relatively new and technically challenging procedure the role of surgeon experience as evidenced by volume and patient outcomes are critical. In this PhD thesis we plan to evaluate survival outcomes of early stage lung cancer patients treated with segmentectomy versus wedge; evaluate differences in outcomes among lung cancer patients undergoing VATS segmentectomy versus open segmentectomy; and evaluate the volume-outcome relationship for surgeons performing VATS.
