This book represents tangible results of the Institute of Applied Physiology and Medicine's objectives of bringing into clinical application the concepts and results of bioengineers and physiologists. lt has grown out of our experience in development of original Doppler diagnostic instrumentation and its application to more than 6,000 patients in non-invasive diagnosis of the extracranial cerebrovascular circulation. It gath ers together bioengineering and physiological concepts which were initially developed for a graduate course for physicians and specialized medical tech nicians. lt should be of particular interest to physicians and medical technol ogists whose specialties lie in the field of neurology, vascular surgery, and radiology. The particular selection of techniques on our complete cerebrovascular non-invasive evaluation was chosen primarily to provide an integrated approach relying primarily on Doppler ultrasonic audio signals, but in addition utilizing a pertinent history and physical examination of the patient. The examination is performed by a specially trained physiology technician. Suffi cient data is provided to the interpreting physician who develops a consulta tive report for the referring physician. The ultimate objective is prevention of stroke and symptoms of cerebrovascular insufficiency. The Doppler functional information supplements the angiography anatomical management decisions for angiography and surgery, anti-platelet or anti-coagulant medication, or to follow the patient with no special treatment.

HIGHLY COMMENDED by the BMA Medical Book Awards for Neurology! Neurosonology and Neuroimaging of Stroke: A Comprehensive Reference Neurosonology is a first-line modality in the diagnosis and management of cerebrovascular disease and especially of stroke. In this new edition of Neurosonology and Neuroimaging of Stroke, this noninvasive, realtime imaging method has been given expanded coverage, particularly for its clinical utility. As in the first edition, the new edition offers both a clear overview of the principles of neurosonology and a casebook exploring critical cerebrovascular problems. Ultrasound anatomy, technical aspects of clinical application, and the advantages and limitations of ultrasound are reviewed and contrasted to conventional, magnetic resonance, and computed tomography angiography. Forty-five selected cases from the authors' extensive collections at Charite - Universitatsmedizin Berlin and the Center of Neurology in Bad Segeberg, Germany, are then discussed. The patient histories and working diagnoses are followed by detailed assessments of the extra- and intracranial color-coded duplex sonographic findings and additional diagnostic procedures. The relevant clinical aspects are presented in a compact, comprehensible way, and for the majority of the cases videos are available in the Thieme MediaCenter, providing further in-depth understanding of the full potential of the method. Every few years a dissertation comes to the area of clinical application of medical technology which carries us forward as on a magic carpet into new regions of understanding and patient care. This book is such a magic carpet. It brings together, in a clear and incisive fashion, important hemodynamic principles with a simple noninvasive method of application to a part of the cerebral vasculature which has been relatively inaccessible. To the lucky and perceptive person who reads this book, a feeling of excitement and hope for progress is engendered. The diligent application of the potentials of transcranial Doppler ultrasound brings new power to our efforts in understanding the cerebral circulation and the causes, treatment and prevention of cerebrovascular disorders. Merrill P. Spencer, M. D. Director Institute of Applied Physiology and Medicine Seattle, Wash. , July 1986 Acknowledgements I am greatly indebted to Prof. He1ge Nornes, Oslo, who introduced me to the fascinating study of cerebral hemodynamics in the early 1970's and since then continually encouraged my interest in this field. It was through his pioneering work on the cerebral circulation-using peroperative electromagnetic flowmetry and Doppler techniques-that the basis was laid for the noninvasive trans cranial approach to the circle of Willis described in this book. I also gratefully acknowledge the stimulating case discussions with Prof. Peter Huber, Berne, at the very early introduction of trans cranial Doppler, the inspiring exchange of ideas with Dr. Merrill P.
