Introduction : Les fractures de l'acétabulum sont actuellement en augmentation, notamment chez le sujet âgé. Il est admis que la voie d'abord dépend directement du type de fracture à prendre en charge. Ainsi, pour répondre aux demandes spécifiques de certaines fractures notamment l'impaction de la lame quadrilatère des voies d'abord intra pelvienne ont vu le jour. Le but de ce travail était de comparer les résultats obtenus en fonction de la voie d'abord utilisée. Méthode : Il s'agit d'une étude comparative rétrospective et monocentrique menée sur une population de fracture prises en charge entre 2015 et 2022 au groupe hospitalier Paris St Joseph. Quatre voies d'abord ont été étudiées : Ilio-inguinale, Stoppa, Stoppa + fenêtre externe et ilio-Stoppa. L'objectif principal était l'évaluation de la réduction via le score de St Joseph. Les critères de jugement secondaires comportaient les complications opératoires. Résultats : La population était composée de 203 fractures avec une prédominance du sexe masculin. Les accidents de la voie publique représentaient la première cause de traumatisme et le type de fracture majoritairement retrouvée était la colonne antérieure et hémi-transverse postérieure. Il n'a pas été retrouvé de différence significative entre les différentes voies d'abords concernant le score de Saint Joseph. L'évaluation des critères de jugement secondaires retrouvent une prévalence plus importante de lésion du nerf cutané latéral de la cuisse et un saignement per opératoire plus important dans le groupe ilio-Stoppa. Si l'étude aux cas par cas des voies ne permet pas d'objectiver une différence, l'étude d'un groupe composé de la voie de Stoppa et ses variantes a permis de mettre en évidence une amélioration du score de post opératoire de manière significative. DISCUSSION : La population étudiée semble identique aux populations retrouvées dans la littérature. L'absence de différence significative concernant le critère de réduction et un saignement per opératoire plus important lors de la réalisation des voies extra-péritonéales semble correspondre aux données fournis par la littérature. Les atteintes du nerf cutané latéral de la cuisse étaient également plus fréquentes dans le groupe ilio-Stoppa. En revanche, la voie de Stoppa et ses variantes permettent une meilleure adaptabilité per opératoire et sont corrélées à une amélioration du score post opératoire. L'absence de score fonctionnel, l'hétérogénéité des groupes concernant l'utilisation de l'OARM et des plaques consoles constituent les principaux biais de cette étude. Conclusion : La voie de Stoppa modifiée, seule ou combinée pourrait bientôt détrôner la voie ilio-inguinale dans la prise en charge des fractures de l'acétabulum. Même si cette étude ne permet pas clairement de définir la place de l'ilio-Stoppa, elle représente la première étude la comparant aux autres voies de la colonne antérieure et à utiliser le score de Saint Joseph comme critère de réussite de l'ostéosynthèse. D'autres études seront nécessaires pour affiner les choix des fenêtres à combiner en fonction du type de fracture.

It is to the great and lasting credit of LORENZ BOHLER and his school that they have in the last decade developed and demonstrated so thoroughly the techniques for the conservative management of fractures. Nevertheless there have always been many, including some from BOHLER'S school, who have found considerable place for surgical management, and with the significant progress in general surgery seen in postwar years, a new stimulus has been given to this part of traumatic surgery, especially since bone injuries have become more complex and frequent. The concept of internal fixation is not new. The serious criticisms that have been levelled at it retain today their basic significance. Progress in the fields of asepsis, corrosion-free metal implants, operative experience and postoperative care has diminished the dangers but has not relieved the surgeon of responsibility.
