In recent years, a number of chronic diseases have been linked, in some cases definitively, to an infectious etiology: peptic ulcer disease with Helicobacter pylori, cervical cancer with several human papillomaviruses, Lyme arthritis and neuroborreliosis with Borrelia burgdorferi, AIDS with the human immunodeficiency virus, liver cancer and cirrhosis with hepatitis B and C viruses, to name a few. The proven and suspected roles of microbes does not stop with physical ailments; infections are increasingly being examined as associated causes of or possible contributors to a variety of serious, chronic neuropsychiatric disorders and to developmental problems, especially in children. The Infectious Etiology of Chronic Diseases: Defining the Relationship, Enhancing the Research, and Mitigating the Effects, summarizes a two-day workshop held by the Institute of Medicine's Forum on Microbial Threats to address this rapidly evolving field. Participants explored factors driving infectious etiologies of chronic diseases of prominence, identified difficulties in linking infectious agents with chronic outcomes, and discussed broad-based strategies and research programs to advance the field.

In 1949 the U.S. National Cancer Institute (NCI) and the Canadian Department of National Health and Welfare (DNHW) commissioned a film, eventually called Challenge. Science Against Cancer, as part of a major effort to recruit young scientists into cancer research. Both organizations feared that poor recruitment would stifle the development of the field at a time when funding for research was growing dramatically. The fear was that there would not be enough new young scientists to meet the demand, and that the shortfall would undermine cancer research and the hopes invested in it. Challenge aimed to persuade young scientists to think of cancer research as a career. This book is the story of that forgotten film and what it tells us about mid-twentieth century American and Canadian cancer research, educational filmmaking, and health education campaigns. It explores why Canadian and American health agencies turned to film to address the problem of scientist recruitment; how filmmakers turned such recruitment concerns into something they thought would work as a film; and how information officers at the NCI and DNHW sought to shape the impact of Challenge by embedding it in a broader educational and propaganda program. It is, in short, an account of the important, but hitherto undocumented, roles of filmmakers and information officers in the promotion of post-Second World War cancer research.

Noncommunicable diseases (NCDs) are by far the major cause of death in lower-middle, upper-middle, and high-income countries; by 2015, they will also be the leading cause of death in low-income countries. In addition to mortality, NCDs account for nearly half of the disease burden measured in disability-adjusted life years (DALYs) in low- and middle-income countries, and large increases in NCD-related DALYs are projected. Addressing this challenge will require policy makers to design and implement economic, health, and social policies to address the links between NCDs and poverty and to minimize the health and economic losses among the population. Public Policy and the Challenge of Chronic Noncommunicable Diseases provides a framework that policy makers can use to formulate their strategies. The authors point out that the most effective policy response will be twofold: to develop programs to avoid the looming NCD burden of disease to the extent possible-for example, through public health interventions and improved health care-and simultaneously to prepare to address the health system and economic pressures that will arise from the increase in NCDs due to the aging of populations. This book will be of interest to governments, international organizations, universities, and research institutions focusing on health care, economic policy, public health, and poverty reduction strategies. Millions of Ghanaians live with diabetes, hypertension, stroke, cancers and other major chronic diseases. Millions more are at risk of getting these conditions. Individuals living with chronic conditions experience many disruptions, especially at the early stages of diagnosis and adjustment. The disruptions are physical (medical complications), psychological (depression), material (impoverishment), social (stigma) and spiritual (struggles with faith and trust). These experiences have an impact on family life and resources, with primary caregivers bearing similar disruptions to their chronically ill loved ones. While chronic conditions cannot be cured, many individuals hope for a cure. This hope drives healthcare seeking across different sectors of Ghana’s vibrant pluralistic health system. When ‘hope for a cure’ meets ‘claims to cure’ within the herbalist and faith healing sectors, especially, the outcomes for individuals and their families can be catastrophic. The Ghanaian situation is mirrored in many African countries. It is estimated that African chronic non-communicable disease (NCD) prevalence, morbidity and mortality rates will rise faster than rates in Asia and Latin America over the coming decades. The long term and costly nature of NCDs has major implications for individuals, communities, health systems and governments. In this inaugural lecture, Professor Ama de-Graft Aikins discusses the psychology of chronic disease risk, experience and care in Africa. She makes a case for why the problem of NCDs needs to be examined through a psychological lens. She draws on her independent and collaborative work on diabetes representations and experiences among Ghanaians in Ghana and Europe, and the broader African and global health literature, to highlight the complex multi-level context of chronic disease risk, experience and care. She presents a synthesis of the evidence through the concepts of physical ills and ideological ills, arguing that both are interconnected and, as a result, must be addressed through interdisciplinary approaches. She concludes by offering practical solutions for reducing chronic disease risk and improving the quality of long-term experience and care in Ghana, using examples from countries that have implemented successful NCD interventions.

On June 11 and June 12, 2019, the National Academies convened a workshop to explore the growing understanding of how the interplay between humans and microbes affects host physiology and causes noncommunicable diseases. Discussions included an overview of colliding epidemics, emerging research on associations between infectious and noncommunicable diseases, risks posed by chronic diseases to the development and severity of infectious diseases, and the influence of the microbiome. Workshop participants also examined the challenges and opportunities of convergence, the integration of health care delivery models and interventions, potential approaches for research, policy, and practice in the immediate-term, and potential directions for the long-term. This publication summarizes the presentations and discussions from the workshop.
