This book aims to present the first comprehensive synthesis of the context and impact of non-communicable diseases (NCDs) in low- and middle-income countries (LMICs) and to offer insights on successful and sustainable interventions and policies that work for at-risk populations. It includes 12 chapters divided into 3 parts. Part I focuses on the state of the problem and state of knowledge on the epidemiology and burden of the major NCDs. Three chapters review the epidemiology and burden of cardiovascular diseases and diabetes (Chapter 1), cancers (Chapter 2) and neurodegenerative diseases such as dementia and Parkinson's disease (Chapter 4). Two chapters focus on the co-morbid and multi-morbid interactions between the major NCDs and infectious diseases like HIV, tuberculosis and malaria (Chapter 3) and mental health disorders (Chapter 5). Part II focuses on best practices and innovation in research and intervention. Four chapters discuss key issues on this theme including health systems strengthening (Chapter 6), population surveillance (Chapter 7), community-based interventions (Chapter 8) and self-help approaches to NCD care (Chapter 9). Part III focuses on policy development and implementation. Three chapters offer a comprehensive analysis of existing policies relevant to NCD prevention and control. They focus on policies that work, as well as discussing the lessons that can be learned from infectious disease control (Chapter 10), NCD control in high-income countries (Chapter 11) and the current policy issues and activities arising from the 2011 UN High Level Meeting on NCDs and leading to a post-2015 global health agenda (Chapter 12).

Noncommunicable diseases (NCDs) are by far the major cause of death in lower-middle, upper-middle, and high-income countries; by 2015, they will also be the leading cause of death in low-income countries. In addition to mortality, NCDs account for nearly half of the disease burden measured in disability-adjusted life years (DALYs) in low- and middle-income countries, and large increases in NCD-related DALYs are projected. Addressing this challenge will require policy makers to design and implement economic, health, and social policies to address the links between NCDs and poverty and to minimize the health and economic losses among the population. Public Policy and the Challenge of Chronic Noncommunicable Diseases provides a framework that policy makers can use to formulate their strategies. The authors point out that the most effective policy response will be twofold: to develop programs to avoid the looming NCD burden of disease to the extent possible-for example, through public health interventions and improved health care-and simultaneously to prepare to address the health system and economic pressures that will arise from the increase in NCDs due to the aging of populations. This book focuses on chronic non communicable diseases (NCDs) in Low Middle Income Countries (LMIC). Currently, NCDs appear to be permeating the globe, with an increasing trend in LMIC. The increasing prevalence of NCDs in these countries is attributed to rapid urbanisation, population ageing, globalisation and industrialisation, all of which result in marked changes in patterns of consumption of food and alcohol, increased tobacco use and sedentary lifestyles, high levels of stress and low levels of physical activity. The burden of NCDs is likely to increase tremendously over the coming decades unless there is appropriate action taken to address the risk factors.

Noncommunicable diseases (NCDs) are the main cause of death worldwide. In addition, the incidence, prevalence, and death rates from NCDs are expected to increase in the future. No single discipline can address the issues of prevention, early detection, access to care, and appropriate treatment that are needed to improve outcomes for NCDs; this growing need will require transdisciplinary collaborations. Improving outcomes in NCDs is clearly a difficult endeavor that will require significant efforts. However, previous research and program implementation indicate that improving outcomes for NCDs is an achievable goal. Given the tremendous impact of NCDs on morbidity, mortality, and costs worldwide, there is little doubt that efforts to improve NDC outcomes are much needed. Recognizing the importance of new strategies and interventions addressing NCDS globally, RTI funded the projects described here to assess how the institute’s range of expertise could be focused on the reduction of NCDs. The projects described in this book focus on the risk factors or treatment (or both) of one or more of the four NCDs that are responsible for the greatest impact on morbidity and mortality: cardiovascular disease, cancer, diabetes, and chronic respiratory diseases. The pilot projects were carried out in low- and middle-income countries, which are disproportionately affected by NCDs. The lessons learned from these initial pilot projects have great potential to be increased in their scale and reduce the devastating impact of noncommunicable diseases.

An estimated 36 million people die from such diseases each year; this represents roughly two out of three deaths globally. Eighty percent of these fatalities occur in developing countries. The statistics are staggering, yet millions of these deaths are preventable. This is an urgent global health issue that demands analysis of gaps in NCD research, new policies and practices, and actionable recommendations to close the gaps. The Johns Hopkins Institute for Applied Economics, Global Health, and the Study of Business Enterprise convened an NCD Working Group of leading scholars to examine a wide range of issues that both the private and public sectors must address to make sustainable progress in NCD prevention and treatment in lower- and middle-income countries. Cardiovascular disease (CVD), once thought to be confined primarily to industrialized nations, has emerged as a major health threat in developing countries. Cardiovascular disease now accounts for nearly 30 percent of deaths in low and middle income countries each year, and is accompanied by significant economic repercussions. Yet most governments, global health institutions, and development agencies have largely overlooked CVD as they have invested in health in developing countries. Recognizing the gap between the compelling evidence of the global CVD burden and the investment needed to prevent and control CVD, the National Heart, Lung, and Blood Institute (NHLBI) turned to the IOM for advice on how to catalyze change. In this report, the IOM recommends that the NHLBI, development agencies, nongovernmental organizations, and governments work toward two essential goals: creating environments that promote heart healthy lifestyle choices and help reduce the risk of chronic diseases, and building public health infrastructure and health systems with the capacity to implement programs that will effectively detect and reduce risk and manage CVD. To meet these goals, the IOM recommends several steps, including improving cooperation and collaboration; implementing effective and feasible strategies; and informing efforts through research and health surveillance. Without better efforts to promote cardiovascular health, global health as a whole will be undermined.
