TB is the most common opportunistic disease and cause of the death for those infected with HIV. Diagnosis of TB in HIV infected patients may be delayed because of atypical clinical presentation and involvement of inaccessible sites and low sputum smear positivity. Further, there has been an increase in rates of drug resistant tuberculosis, including multi-drug (MDR-TB) and extensively drug resistant TB (XDR-TB), which are difficult to treat and contribute to increased morbidity and mortality. TB is the most common serious opportunistic infection in HIV positive patients and is the manifestation of AIDS in more than 50% of cases in developing countries. TB shortens the survival of patients afflicted with HIV infection, may accelerate the progression of HIV, and is the cause of death in one third of people with AIDS worldwide. While HIV and TB can individually be the major causes for public health threats, the combination of the two has proven to have a far greater impact on the epidemiologic progression and consequently on the global health scene. Although the increased access to highly active antiretroviral therapy (HAART) has led to a reduction in HIV-associated opportunistic infections and hence mortality, but the concurrent management of HIV/TB co-infection remains a serious challenge to the health care delivery system. Discussion on the mechanisms produced by infectious cofactors with impact on disease pathology could shed light on how to design potential interventions that could decelerate disease progression. It is the need of the hour to design strategies against HIV-TB co-infection. This book is based on my PG thesis work in Medical Microbiology written long bank, but in the current scenario the topic of TB-HIV co-infections holds relevance and is currently a burning issue. This has motivated me to convert my thesis in the form of a Book. The knowledge particularly in the field of medical sciences should not be restricted just to earn a University degree but should be disseminated in the entire scientific community so that medical fraternity including teachers, students, doctors, paramedics, researchers and above all the patients are benefitted. I am thankful to publications for giving me an opportunity to fulfil this dream.

BACKGROUNDS AND AIMS Pinworm infection, also called enterobiasis, is caused byEnterobius vermicularis. E vermicularisis a white slender nematode with a pointed tail.In humans, they reside in the cecum, appendix and ascending colon. Transmission can occur via direct contact with contaminated furniture, bedclothes, bedding, towels, toilets, doorknobs or other objects.Pinworm infection is primarily a pediatric condition. Of all age groups, school-aged children (aged 5-9 years) are most at risk for pinworm infections. Incubation period is usually 1u20132 months. Infectious mononucleosis, refers to a group of symptoms usually caused by the Epstein-Barr virus (EBV). u0422he virus is spread through direct contact with saliva from the mouth of an infected person or other bodily fluids such as blood.The virus has an incubation period of approximately four to eight weeks (1-2 months). The following groups have a higher risk for getting mono: young people between the ages of 15 and 30, students, medical interns, nurses, caregivers and people who take medication that suppress the immune system.METHODS A 8 year old girl came on exam for symptom such as itching in the anal region, abdominal pain in last 2 months and the presence of worms in the stool. Physical examination revealed diffuse abdominal pain. Blood test were negative for leucocyte and CRP. First laboratory findings revealed normal hemoglobin, leukocytes and serum C-reactive protein and normal urine analysis. The crop culture was positive for Enterobius vermicularis. She received 3 days therapy with mebendazole.Seven days after therapy, she came on exam with high fever, sore throat, swollen submandibular lymph nodes and abdominal pain. The liver and spleen werenu2019t palpable under the right costal margin.RESULTS Ultrasonography of the abdomen confirmed normal liver and gallbladder, enlarged spleen for the age (140 mm), normal findings on kidneys and bladder. Crop culture was positive for Enterobius vermicularis. First laboratory findings revealed normal hemoglobin, leukocytes and serum C-reactive protein and normal urine analysis. Second laboratory findings revealed: leukocytes 12,4 x10^9/L with 14% neutrophils, 80% lymphocytes. Transaminases were elevated: AST 131u202689 U/l, ALT 153...123 U/l. LDH 1375 U/L; CRP 18 mg/dl. Test for Epstein Barr virus and Cytomegalovirus were positive. CONCLUSIONS Pinworm infection and infectious mononucleosis are two different infectious disease caused by two completely different infectious microorganisms. The only similar and common characteristic is the same incubation period. Still, in this case, remains the question, is it the drop of the immune system the reason that made this girl sensitive to these different microorganism, or still, there is some connection between these two diseases.REFERENCES1.Johannsen EC Schooley RT Kaye KM.
