The story of Depo-Provera joins the national struggle over the drug's FDA approval to the state legal issues raised by its contraceptive and criminal justice uses. Depo-Provera is known as an injectable hormonal birth control method, but few are familiar with its dark and complicated history. Depo-Provera was tested on women since the mid-1960s without their informed consent until it was FDA-approved in 1992, but never FDA-approved as chemical castration for male sex offenders. Contraceptive Risk is William Green's landmark study of Depo-Provera. "Concerns about the safety of the pill have come to the fore recently, due in large part to the public outcry over deaths allegedly caused by popular contraceptives Yaz and Yasmin. Media reports link the drugs to at least 23 deaths in Canada and over 100 in the U.S. as well as thousands of injuries worldwide, as a result of a purported increased incidence of blood clots. The question of what should be considered an acceptable risk threshold for approved contraceptive pills has been contested by many groups, including regulatory bodies, medical professionals, epidemiologists, and consumers. This thesis provides an in-depth sociological examination of the Yaz/Yasmin controversy through qualitative analysis of stakeholder interviews and content analysis of key medical, regulatory, and legal documents. I examine the most recent safety debates in order to analyze the central phenomena that play a role in the regulation and risk/benefit assessment of oral contraceptives: managing data uncertainty, calculating reproductive risks against ancillary benefits, and the precariousness of informed consent. After an introductory chapter in which I situate the thesis within the larger scholarly conversations on contraceptive assessment specifically and technological risk more generally, the findings are presented in three article-length manuscripts. In the first article, I investigate strategies that regulatory bodies and professional associations use to assure the public of the safety of Yaz and Yasmin. I found that a discourse of pregnancy risks is employed in official risk communication. I argue that this has gendered implications for the development of contraceptives and their risk assessment. In the second article, I explore how pill users who have experienced side effects understand the risks of hormonal contraception and advocate for changes in risk communication and drug regulation. I found that women highlighted the inadequacy of risk information received from both doctors and pharmaceutical companies. In addition, affected users rejected the use of reproductive risk as the primary comparative risk when assessing pill safety. In the third paper, I look at processes through which individual stakeholders measure and debate contraceptive risk. Here, I highlight how contraceptive risk assessment is characterized by systemic uncertainty and doubt. Furthermore, the paper stresses the tough choices that users have to make in a climate of indecision coupled with pharmaceutical companies' involvement in research and marketing. While we know that the history of the pill has been fraught with many debates about its safety, less attention has been paid to the current changing context and how different social actors still contest the measurement and meaning of contraceptive risk. I found that many questions about risks are unresolved or dealt with in ways that create divergence between professionals and affected users. In the concluding chapter, I discuss the sociological implications of my findings, the limitations of this study and potential directions for future research on the evaluation of contraceptive risk. The Yaz/Yasmin controversy has amplified contemporary critiques of the pill. In documenting and analyzing the dynamics of this risk debate, this thesis contributes to a deeper understanding of how the risks and benefits of contraception are currently assessed in the North American context." --

A. A. Haspe/s It is with pleasure that I welcome you, on behalf of Professor Rolland and myself, to Amsterdam for this International Symposium on 'Benefits and Risks of Hormonal Contraception'. As a means offamily planning the pill is about 25 years old - a timespan which has been characterized by an enormous increase in public interest and concern with family health and family-planning. Undoubtedly we have learned a lot over the last 25 years. As you see in Figure 1, in the seventies in Holland relatively more fertile women used the pill than in any other country in the world. In 1974 new combination pills were introduced containing less than 50 JAg of ethinyl estradiol. In 1981 50 % of Dutch pill users took a sub-50 (Figure 2). The same is true for the Scandi navian countries. In our own University Clinic 95 % of pill-users take a sub-50 pill; only 5 % use a 50 JAg pill on medical indication. This decrease in estrogen dosage, which is usually accompanied by a decrease of progestational component as well, has resulted in a decrease of thromboembolic disease. Factors that are still important to consider are diabetes mellitus, hypertension, adipositas and smoking. Good selection of patients together with the prescribing where possible of sub-50 pills may result in the numbers of compli cations and side-effects being close to those encountered in the control group.

Women with chronic medical problems are at higher risk for complications during pregnancy and therefore, they are especially in need of appropriate preconception and contraception care. Furthermore, many women with chronic medical problems do not obtain adequate preconception and contraception care. Despite published guidelines by the Centers for Disease Control and Prevention, there is a substantial gap in medical practice regarding the use of contraception in women with co-existing medical problems. Contraception for the Medically Challenging Patient fills the gap that currently exists in the knowledge of correct contraceptive prescribing practice and shows that inappropriate contraindications can easily become a barrier to effective contraception use among women. Chapters highlight obsolete views about appropriate candidates for contraception and address the complex contraceptive needs of today's medically challenging patients with HIV/AIDS, uterine fibroids or cardiac, neurologic or thyroid disease. The book gives attention to recommendations on the use of contraception in women with medical problems such as diabetes, obesity, epilepsy, and lupus, among others and provides comprehensive information regarding the effects that certain drugs may have on contraceptive hormone levels. While national guidelines do exist for contraceptive eligibility, this book discusses in more detail the evidence behind the guideline recommendations and the nuances that clinicians confront in daily practice.

This book analyses the reasons for relying on behavioural contraception methods among urban ‘elites’ in India and examines their efficacy in controlling fertility. It also traces variations in contraception choice over the reproductive cycle of women. Although researchers and policy makers generally equate reliance on behavioural contraceptive methods with low levels of education and awareness and lack of desire to control fertility, this perception has been questioned in recent years. The authors’ analysis of the first three rounds of the National Family Health Survey (NFHS) data in India reveals that behavioural contraceptive methods are popular in eastern India. Moreover, it is urban educated women who rely on behavioural methods, and are apparently able to regulate fertility quite effectively with such methods. NFHS data, however, has some limitations and this motivates the authors to explore birth control methods through primary surveys of currently married graduate women in Kolkata. The use of behavioural contraception methods is a little researched area globally and this is the first book focusing on the topic in India.

Evaluates the strength and significance of evidence linking use of combined oral contraceptives to an increased risk of cardiovascular disease in women. Recommendations and conclusions reflect the consensus reached by a group of scientists, including researchers directly involved in several recent large-scale investigations of cardiovascular disease and hormone contraception. The experts also considered evidence that other factors, such as smoking, hypertension, diabetes, age, and family history, might influence the risk of cardiovascular disease in users of combined oral contraceptives. In view of major recent changes in the hormonal content of these contraceptives and the prescribing patterns of providers, the assessment concentrates on data collected after 1980. The experts also aimed to determine whether the risk of disease is influenced by the estrogen or progestogen content of different preparations. To facilitate the comparison and interpretation of study results, the report opens with a discussion of the strengths and weaknesses of the different epidemiological approaches used to assess the safety of steroid contraceptives. Also discussed is the important distinction between the use of relative risk to examine epidemiological associations and the use of absolute risk to examine clinical importance. Against this background, the main part of the report examines the strength of evidence linking steroid contraceptives to an increased risk of acute myocardial infarction, ischaemic stroke, haemorrhagic stroke, and venous thromboembolism. Studies of combined oral contraceptives and progestogen-only contraceptives are considered separately for each disease. An effort is also made to determine whether conclusions, based on studies conducted in industrialized countries, are also valid for women living in the developing world. The assessment of findings from epidemiological studies is complemented by a review of possible biological explanations for the effects of combined oral contraceptives on cardiovascular functions. On the basis of all available evidence, the report concludes that mortality rates from cardiovascular disease are extremely low among women of reproductive age and that the added risk of using steroid contraceptives is also very low in users who do not smoke or have other risk factors for cardiovascular disease. A final section places these risks in perspective and offers balanced advice useful to providers of family planning when helping women to make informed choices about the use of hormonal contraceptives.
