When Handbook of Normative Data for Neuropsychological Assessment was published in 1999, it was the first book to provide neuropsychologists with summaries and critiques of normative data for neuropsychological tests. It has long been appreciated among clinical neuropsychologists that both primary and secondary factors contribute to cognitive dysfunction in neurological patients. Primary influences are the direct result of the extent and location of damage to the brain. Secondary influences stem from something associated with brain injury or disease besides the specific areas of the brain affected. For example, a patient with a neurological disease may develop depression, something which in turn often impacts cognitive functioning. Other secondary factors associated with disease besides depression can also negatively impact cognitive functioning, including anxiety, pain, fatigue, and motor impairments, to name a few. Despite the widespread appreciation of the importance of such secondary factors on cognitive functioning in clinical neuropsychology, there has never been a single source for this information that is readily available to clinicians and researchers. The present volume provides such a source, using an evidence-based framework comprised of two broad sections. The first section includes chapters that comprehensively address particular secondary influences, independent of any particular neurological disorder. For example, there are chapters on the impact on cognitive functioning of depression, anxiety, fatigue, pain, diagnosis threat, and symptom invalidity. The second section has chapters that focus on specific neurological conditions and the most salient secondary factors that need to be considered in these conditions. Multiple Sclerosis, HIV, Parkinson's Disease, Traumatic Brain Injury, Alzheimer's Disease/MCI/Stroke, and Epilepsy are all considered. The chapters include vivid case studies that illustrate the principles outlined in the chapters to help clinicians consider how such principles can apply to particular patients. One of the challenges the field of clinical neuropsychology faces is to develop an assessment process that is relevant and responsive to the needs of patients. Indeed, it has been suggested that the survival of neuropsychology as a clinical discipline may be threatened due to an overemphasis on diagnosing cognitive deficits as opposed to developing methods that meet patients’ needs. One way to meet this need is for psychologists to extend their services by developing clinical interventions that contribute to enhancing patients’ cognitive and emotional well-being. Providing feedback from the results of neuropsychological tests is one method suggested as a way to enhance patient care and satisfaction with the assessment process while providing a link between clinical assessment and therapeutic interventions. In current practice, providing feedback to patients about the results of neuropsychological assessments has been considered an optional procedure by clinicians and thus received little attention in the literature. Yet there is evidence that when feedback is provided to patients the effects are overwhelmingly positive. Feedback provides a bridge between assessment and treatment by facilitating the development of applicable treatment plans. There is no agreed upon conceptual framework for providing feedback from neuropsychological assessment, although researchers have made recommendations. However, these recommendations have done little to affect clinical practice and training despite the fact that including patient-oriented feedback enhances the likelihood that neuropsychological assessment will remain a relevant and important component of patient care. The purpose of this proposed book is to first describe developments in methods of neuropsychological assessment feedback that involve active collaboration with patients in an open exchange of information and results. Second, we will present a comprehensive model for conducting neuropsychological assessment feedback. The authors of this book are expert clinical practitioners and academic researchers who are at the forefront of the development and implementation of Collaborative Therapeutic Neuropsychological Assessment methods. Ecological validity, the functional and predictive relationship between a patient's performance on a set of neuropsychological tests and their behavior in a variety of real-world settings, is emerging as a powerful tool. Since neuropsychological data were not designed to be ecologically valid, attempts to use the data in such a manner have been filled with pitfalls. The authors present a strong argument for reevaluating existing tests and/or developing new measurements. Special attention is given to specific functions such as the cognitive constructs of attention, perception, working memory, language, and executive function. Additional consideration is given to the assessment of special populations: children, minorities, substance abusers, elderly, and emotionally disturbed neurological populations.
